结肠癌组织中miR-340和CCND1表达及临床意义 被引量:11

Expression and clinical significance of miR-340 and CCND1 in colon cancer

摘要 目的 检测结肠癌组织和癌旁正常肠黏膜组织中miR-340和细胞周期蛋白D1(CCND1)表达,探讨其与结肠癌临床病理特征和预后的关系。方法 选择2013年1月至2014年12月间接受结肠癌根治术的87例初治患者。采用实时荧光定量PCR(QPCR)检测miR-340表达水平,采用免疫组化染色法检测CCND1蛋白表达水平。分析其表达与结肠癌临床病理特征(年龄、性别、肿瘤直径、临床分期、T分期、淋巴结转移、脉管侵犯、Ducks分期、肿瘤位置、分化程度等)的关系。采用Kaplan-Meier法绘制生存曲线,生存差异行Log-rank检验。结果 结肠癌组织中miR-340的表达水平为1.53±0.60,低于癌旁组织的2.75±0.62,差异有统计学意义(P<0.05);结肠癌组织中CCND1蛋白阳性表达率为59.77%(52/87),高于癌旁组织的41.38%(36/87),差异有统计学意义(P<0.05)。结肠癌组织中miR-340表达和CCND1蛋白表达呈负相关性(r=-0.408,P<0.001)。miR-340表达与肿瘤直径、脉管侵犯、Ducks分期、临床分期、淋巴结转移有关(P<0.05),CCND1表达与肿瘤直径、肿瘤位置、Ducks分期、临床分期有关(P<0.05)。87例结肠癌患者5年生存率为59.77%(52/87)。miR-340低表达者的5年生存率为47.73%(21/44),低于高表达者的72.09%(31/43),差异有统计学意义(P<0.05)。miR-340低表达+CCND1阳性表达者5年生存率为31.43%(11/35),低于非miR-340低表达+CCND1阳性表达者的78.85%(41/52),差异有统计学意义(P<0.05)。结论 miR-340表达与肿瘤直径、脉管侵犯、Dukes分期、临床分期、淋巴结转移以及总生存有关,且与CCND1表达呈负相关;miR-340联合CCND1有望成为判断结肠癌患者预后的重要指标。 Objective To detect the expression of miR-340 and Cyclin D1 (CCND1) in colon cancer tissues and adjacent normal intestinal mucosa,and to explore its relationship with clinicopathological features and prognosis of colon cancer.Methods Eighty-seven patients who underwent radical resection of colon cancer from January 2013 to December 2014 were selected.Real-time fluorescence quantitative PCR (QPCR) was used to detect the expression of miR-340,and immunohistochemical staining was used to detect the expression of CCND1.The relationship between the expression and clinicopathological features (age,sex,tumor diameter,clinical stage,T stage,lymph node metastasis,vascular invasion,Ducks stage,tumor location,differentiation degree,etc.) was analyzed.The survival curve was drawn by Kaplan-Meier method,and the survival difference was tested by Log-rank test.Results The expression level of miR-340 in colon cancer tissues was 1.53±0.60,lower than that in adjacent tissues (2.75±0.62,P<0.05).The positive expression rate of CCND1 protein in colon cancer tissues was 59.77%(52/87),higher than 41.38%(36/87) in adjacent tissues (P<0.05).There was a negative correlation between the expression of miR-340 and the expression of CCND1 in colon cancer (r =-0.408,P<0.001).The expression of miR-340 was correlated with tumor diameter,vascular invasion,Ducks stage,clinical stage and lymph node metastasis (P<0.05).The expression of CCND1 was correlated with tumor diameter,location,Ducks stage and clinical stage (P<0.05).The 5-year survival rate of 87 patients with colon cancer was 59.77%.The 5-year survival rate of miR-340 low-expression group was 47.73%(21/44),which was lower than 72.09%(31/43) in miR-340 high-expression group (P<0.05).The 5-year survival rate of the patients with low expression of miR-340 and positive expression of CCND1 was 31.43%(11/35),which was lower than that of the patients without low expression of miR-340 and positive expression of CCND1 (78.85%(41/52),with significant difference (P<0.05).Conclusion The expression of miR-340 was negatively correlated with the diameter of tumors,vascular invasion,Dukes staging,clinical staging,lymph node metastasis and overall survival,and the expression of CNCD1 was negatively correlated.The combination of miR-340 and CCND1 is expected to be an important prognostic indicator for patients with colon cancer.
