摘要
目的探讨羚羊清肺散联合注射用炎琥宁治疗小儿病毒性肺炎的临床疗效。方法选取2016年12月-2017年12月在成都大学附属医院接受诊治100例小儿病毒性肺炎患儿,所有患儿随机分为对照组(48例)和治疗组(52例)。对照组患儿静脉滴注注射用炎琥宁,将80 mg加入5%葡萄糖注射液200 mL中,1次/d。治疗组患儿在对照组治疗的基础上口服羚羊清肺散,1 g/次,2次/d。两组患儿连续治疗7 d。观察两组的临床疗效,比较两组的体征改善时间、血清学指标。结果治疗后,对照组和治疗组的总有效率分别为79.17%、90.38%,两组比较差异有统计学意义(P<0.05)。治疗后,治疗组患儿退热时间、咳嗽停止时间、痰鸣消失时间、肺部啰音消失时间均明显短于对照组,两组比较差异具有统计学意义(P<0.05)。治疗后,两组患儿血清C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均显著降低,同组比较差异具有统计学意义(P<0.05);且治疗后治疗组患儿血清炎性因子水平明显低于对照组,两组患儿比较差异具有统计学意义(P<0.05)。治疗后,两组患儿血清降钙素原、血小板激活因子、可溶性髓样细胞触发受体1水平均显著降低,同组比较差异有统计学意义(P<0.05);且治疗后治疗组患儿血清因子水平明显低于对照组,两组患儿比较差异有统计学意义(P<0.05)。结论羚羊清肺散联合注射用炎琥宁治疗小儿病毒性肺炎具有较好的临床疗效,能改善患儿临床症状,降低炎性因子水平,安全性较好,具有一定的临床推广应用价值。
Objective To investigate the clinical effects of Lingyang Qingfei Powder combined with Potassium Sodium Dehydroandrographolide Succinate for injection in treatment of viral pneumonia in children. Methods Children(100 cases) with viral pneumonia in the Affiliated Hospital & Clinical Medical College of Chengdu University from December 2016 to December 2017 were randomly divided into the control group(48 cases) and the treatment group(52 cases). Children in the control group were iv administered with Potassium Sodium Dehydroandrographolide Succinate for injection, 80 mg added into 5% glucose solution 200 mL, once daily. Children in the treatment group were po administered with Lingyang Qingfei Powder on the basis of the control group, 1 g/time, two daily. Children in two groups were treated for 7 d. After treatment, the clinical efficacies were evaluated, and the improvement time of physical signs, the levels of serum inflammatory factors and the levels of serum factors in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 79.17% and 90.38%, respectively, and there was difference between two groups(P < 0.05). After treatment, the antipyretic time, the stop time of cough, the disappearance time of phlegm ringing, and the disappearance time of lung rale in the treatment group were shorter than those in the control group, there was difference between two groups(P < 0.05). After treatment, the levels of CRP, IL-6, and TNF-α in two groups were significantly decreased, and the difference was statistically significant in the same group(P < 0.05). And the serum inflammatory factors in the treatment group were significantly lower than those in the control group, with significant difference between two groups(P < 0.05). After treatment, the levels of serum procalcitonin, platelet activating factor, and soluble myeloid cell trigger receptor 1 in two groups were significantly decreased, and the difference was statistically significant in the same group(P < 0.05). And the serum factors in the treatment group were significantly lower than those in the control group, with significant difference between two groups(P < 0.05). Conclusion Lingyang Qingfei Powder combined with Potassium Sodium Dehydroandrographolide Succinate for injection has clinical curative effect in treatment of viral pneumonia in children, can improve the clinical symptoms of children, and reduce the level of inflammatory factors, with good safety, which has a certain clinical application value.
