摘要
目的:研究小儿化脓坏疽阑尾炎腹腔镜术后放置引流的影响及必要性。方法:选取2017年我院收治的93例行腹腔镜阑尾切除术的化脓坏疽阑尾炎患儿,其中2017年1-6月收治放置引流组患者(41例),2017年7-12月收治不放置引流组患者(52例),术后观察及对比患儿预后情况。结果:针对化脓坏疽阑尾炎患儿,放置引流组明显延长术中时间[(103±3.7)min VS(93±4.5)min,P<0.05],增大患儿伤口感染概率(19.04%VS 3.92%,P<0.05),延长愈合时间[(13.4±3.6)d VS(6.2±1.8)d,P<0.05],增加腹腔脓肿的发生率(14.63%VS 1.92%,P<0.05)及延长住院时间[(17.1±1.5)d VS(9.6±1.2)d,P<0.05]。结论:对于阑尾炎尚未穿孔的患儿,手术后不放置引流的效果更优。
Objective:To study the effect and necessity of placement of drainage after laparoscopic surgery for pediatric suppurative appendicitis.Methods:93 children with septic sinus appendicitis who underwent laparoscopic appendectomy in our hospital in 2017 were enrolled. Among them, patients in the drainage group(41 cases) were admitted from January to June 2017,and patients in the not drainage group(52 cases) were admitted from July to December 2017.The postoperative observation and comparison of the prognosis of the children were observed.Results: For children with suppurative appendicitis,the drainage group significantly prolonged the intraoperative time [(103±3.7)min VS(93±4.5)min,P<0.05]and increase the probability of wound infection in children(19.04% VS 3.92%, P<0.05). Meanwhile,it prolonged healing time[(13.4±3.6) days VS(6.2±1.8) days,P<0.05] and increased incidence of abdominal abscess(14.63%VS1.92%,P<0.05).Finally,extend the length of hospital stay[(17.1±1.5)days VS(9.6±1.2)days,P<0.05].Conclusion:For children with appendicitis who have not been perforated,the effect of not placing drainage after surgery is better.
