多层螺旋CT在诊断脑胶质瘤中临床应用价值研究 被引量:8

摘要 目的探讨多层螺旋CT平扫及增强扫描在诊断脑胶质瘤中临床应用价值。方法我院收治的脑胶质瘤患者65例,总结脑胶质瘤在CT平扫及CT三期增强扫描中的图像表现特征,以手术结果为参照标准,计算CT平扫及CT三期增强诊断胶质瘤的敏感度、特异度。结果 CT平扫中,病灶单纯低密度影19例,混杂密度影34例,高密度影12例,其中合并钙化者4例。三期增强扫描中,12例星形细胞瘤患者未见明显强化,27例星形细胞瘤强化明显;环壁厚薄呈现不均匀者17例,65例环壁厚度为4~8 mm,环内CT值18~25 Hu。13例出现环状强化特征,同时伴有囊变液化坏死低密度区,6例出现单纯实质性斑状增强,10例病灶区域呈现花环状强化,实质期病灶内见斑状活毛细血管纹状强化改变。CT平扫诊断脑胶质瘤的敏感度及特异度分别为86. 15%、80. 00%,明显低于CT三期增强扫描98. 46%、95. 38%(P<0. 05)。结论多层螺旋CT平扫及增强扫描均能清晰显示脑胶质瘤的图像特点,其中增强扫描诊断脑胶质瘤敏感度及特异度高,值得临床推广应用。 Objective To explore the clinical application value of multi-slice spiral CT(MSCT) in the diagnosis of glioma.Methods Sixty-five patients with glioma admitted to our hospital were selected.The image features of glioma in CT non-enhanced scan and triple-phase contrast-enhanced scan were summarized.The sensitivity and specificity of CT non-enhanced scan and triple-phase contrast-enhanced scans in the diagnosis of glioma were calculated based on the surgical results.Results In the CT non-enhanced scan,there were 19 cases with simple low-density shadow,34 with mixed density shadow and 12 with high density shadow among which 4 cases were combined with calcification. In triple-phase contrast-enhanced scans,there were 12 patients with astrocytoma without obvious enhancement and 27 with obvious enhancement of astrocytoma.There were 17 cases with uneven thickness of annular wall,65 with 4 ~ 8 mm thickness of annular wall and 18 ~ 25 Hu of intracyclic CT value.In the 13 patients,annular enhancement was observed accompanied by cystic liquefaction necrosis with low-density region.In the 6 patients,pure substantial macular enhancement was observed.In the 10 patients,the lesion area showed floral ring enhancement,and in the parenchymal lesions,mottled living capillary striated enhancement was observed.The sensitivity and specificity of CT non-enhanced scanning in the diagnosis of glioma were 86. 15% and 80. 00%,respectively,that were significantly lower than 98. 46% and 95. 38% of triple-phase contrast-enhanced scans(P< 0. 05).Conclusion Both the non-enhanced scan and the enhanced scan of multi-slice spiral CT can clearly show the image features of glioma.
