摘要
目的建立不同年龄儿童血清促甲状腺激素(TSH)、三碘甲状腺原氨酸(T3)、甲状腺素(T4)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)的参考区间。方法按照美国临床实验室标准化协会(CLSI)EP28-A3C文件,筛选出7 d^12岁健康儿童947例(男性585例,女性362例),按年龄分组(7 d^1岁、2岁~3岁、4岁~5岁、6岁~7岁、8岁~12岁),以非参数法统计各个分组TSH、T3、T4、FT3、FT4的参考区间,同时使用独立样本t检验分析方法比较各个分组内的性别差异。结果 7 d^1岁分组与2岁~3岁分组比较,TSH、T3、T4、FT3、FT4五项的参考区间分布差异均有统计学意义(P<0.05),2岁~3岁及以上各个分组之间TSH、T3、T4、FT3、FT4五项的参考区间分布差异均无统计学意义(P>0.05)。各分组性别之间差异无统计学意义(P>0.05)。结论建立了7 d^12岁健康儿童人群甲状腺激素的参考区间,为临床诊断儿童甲状腺疾病提供诊断依据。
Objective To establish a reference interval of thyroid stimulating hormone(TSH),triiodothyronine(T3),thyroxine(T4),free triiodothyronine(FT3) and free thyroxine(FT4) for children.Methods According to the recommendations of Clinical and Laboratory Standards Institute(CLSI) EP28-A3 C documents,947 persons aged 7-12 years old were recruited(585 males,362 females).These children were classified into 5 groups according to ages(7 days-1 year,2-3 years,4-5 years,6-7 years and 8-12 years).Non-parametric methods were used to establish the reference intervals of TSH,T3,T4,FT3 and FT4.The gender difference in each group was compared by independent-sample-t tests.Results There was statistical significance on the difference in reference interval distributions between 7 days-1 year old and 2-3 years old(P<0.05).While there was no statistical significance on the differences in reference interval distributions between the two groups above 2-3 years old(P>0.05).There was no statistical significance between genders of each group(P>0.05).Conclusion The reference intervals were established for healthy children aged 7 days old-12 years old,which can provide support for the clinical diagnosis of thyroid disease.
