摘要
目的探讨异丙托溴铵联合沙丁胺醇和二羟丙茶碱对小儿哮喘肺功能和气道炎性介质的改善效果.方法选取2014年3月至2017年2月于达州市中心医就诊的3~5岁哮喘患儿130例为研究对象,采用随机数表法将患儿分为观察组(65例)和对照组(65例),观察组患儿在常规治疗基础上采用异丙托溴铵联合沙丁胺醇和二羟丙茶碱治疗,对照组患儿在常规治疗基础上采用沙丁胺醇联合二羟丙茶碱治疗,对比两组患儿治疗前后呼气流量峰值(peak expiratory flow,PEF)、第1秒用力呼气容积(forced expiratory volume in one second,FEV1)、达峰时间比、达峰容积比、呼出气一氧化氮(fractional exhaled nitric oxide,FeNO)水平、诱导痰嗜酸性粒细胞比率以及中性粒细胞比率变化情况.结果治疗后,观察组患儿PEF、FEV1、达峰时间比、达峰容积比均显著高于对照组(均P<0.05),FeNO水平、诱导痰嗜酸性粒细胞比率及中性粒细胞比率均显著低于对照组(均P<0.01).结论异丙托溴铵联合沙丁胺醇和二羟丙茶碱有助于改善患儿的肺功能,降低气道炎性介质水平.
Objective To investigate the effects of Isoproteronium bromide combined with Salbutamol and Dihydroxypropyl theophylline on pulmonary function and airway inflammatory mediators in children with asthma. Method 130 children from aged3 to 5 years old in Dazhou Central Hospital from March 2014 to February 2017 was selected as research objects. They were divided into observation group(65 cases) and control group(65 cases) according to the random. Those in observation group were treated with Isoproteronium bromide combined with Salbutamol and Dihydroxypropyl theophylline on the basis of routine treatment, while those in control group were treated with Salbutamol combined with Dihydroxypropyl theophylline on the basis of routine treatment. The peak expiratory flow(PEF), forced expiratory volume in one second(FEV1), peak time ratio, peak volume ratio, fractional exhaled nitric oxide(Fe NO), eosinophil ratio and neutrophil ratio in induced sputum before and after treatment between the two groups were compared. Result After treatment, PEF, FEV1, peak time ratio and peak volume ratio of observation group were significantly higher than those of control group(all P<0.05), the level of FeNO, eosinophil ratio and neutrophil ratio in induced sputum in observation group were significantly lower than those in control group(all P<0.01). Conclusion Ipratropium bromide combined with Salbutamol and Dihydroxypropyl theophylline in children with asthma are helpful to improve their pulmonary function and reduce the level of airway inflammatory mediators.
