磁共振自由呼吸CS-VIBE在肺部肿瘤动态增强成像中的应用研究 被引量:2

摘要 目的:比较自由呼吸CS-VIBE和屏气TWIST-VIBE在肺病灶成像的图像质量,探讨CS-VIBE在肺部肿瘤动态增强的应用价值。方法:纳入肺肿瘤患者30例(男18,女12),采用CS-VIBE进行自由呼吸状态下多期动态增强扫描(45期,323s),并于延迟期行TWIST-VIBE屏气扫描(2期,20s),扫描完成后,根据病人屏气情况,分为能屏气组及无法屏气组,由2位放射科医生盲法对图像总体质量、病灶边缘锐利度、总伪影等级及诊断自信进行评分。结果:对于能屏气的患者,CS-VIBE和TWIST-VIBE图像质量无统计学差异(P>0.05),但对于无法屏气的患者,CS-VIBE各项评分指标均明显高于TWIST-VIBE (P<0.05)。屏气组与无法屏气组之间CS-V田E扫描图像质量无明显差异(P>0.05)。结论:对于无法屏气的患者,CS-VIBE序列能明显降低肺部呼吸运动伪影,获得较好的图像质量,并能连续多期动态增强扫描,既可满足高时间和空间分辨率,又可获得相关参数进行定量评估。 Purpose: To explore the application value of CS-VIBE in dynamic contrast enhancement (DCE) of lung tumors by comparing the image quality of lung mass using CS-VIBE and TWIST-VIBE. Methods: Thirty patients (18 male, 12 female) with lung lesions underwent free-breathing DCE using a prototype CS-VIBE sequence (45 phases, 5min 21s), immediately followed by a TWIST VIBE sequence (2 phases, 20s) in the delayed phase. Patients were divided by the technician into cooperative group and non-cooperative group according to their ability in successfully holding their breath. Two experienced radiologists blindly scored the overall image quality, lesion delineation, overall artifact level, and diagnostic confidence of the CS-VIBE and the TWIST VIBE images for each case. Results: The image quality of the free-breathing CS-VIBE and TWIST-VIBE sequences were with no significant difference in the cooperative group (P>0.05). In the non-cooperative group, the scores of CS-VIBE were significantly higher than those of TWIST-VIBE (P<0.05). The image quality of CS-VIBE was with no significant difference between the cooperative group and non-cooperative group (P>0.05). Conclusion: For the patients who are unable to hold their breath, CS-VIBE can reduce motion artifacts and obtain satisfactory image quality in free-breathing DCE-MRI of lung cancer with continuous multi-phase scanning of high temporal-spatial resolution for quatative evaluation.
