摘要
目的 探讨血细胞参数在儿童支气管哮喘诊断中的应用价值。方法 选取2018年1—12月来南京医科大学附属无锡人民医院就诊的27例支气管哮喘患儿作为病例组,同期选择年龄、性别相匹配的109名健康体检儿童作为对照组,检测两组受试者外周血细胞参数,统计分析各组间指标差异变化及其在儿童支气管哮喘中的临床诊断价值。结果 与对照组相比,病例组患儿的白细胞(WBC)、嗜酸粒细胞(EO)、单核细胞(MO)、中性粒细胞(NE)、血小板(PLT)、血小板压积(PCT)、血小板平均体积(MPV)显著升高,嗜碱粒细胞(BA)、红细胞平均体积(MCV)显著降低(P<0.05)。WBC、BA、MO、NE、PLT、PCT、MPV、MCV在诊断儿童支气管哮喘中的受试者工作特征曲线下面积(AUC)差异有统计学意义(P<0.05),其中MPV、PCT、BA的AUC分别为0.871、0.830、0.756。在Cut-off取值下,MPV的诊断灵敏度(0.963)最高,PCT的诊断特异度(0.881)最高。结论 支气管哮喘患儿的血细胞参数表现为粒细胞、MO、PLT指标的显著异常,其中MPV、PCT的诊断价值较高,可能作为辅助诊断支气管哮喘的潜在指标,但可靠性有待进一步研究。
Objective To explore the changes and clinical value of blood cell parameters in children with bronchial asthma.Methods Blood cell parameters of 27 children with bronchial asthma(case group) and 109 healthy children(control group) matched by age and sex were measured.Statistical analysis of the difference between the two groups and the clinical diagnostic significance in children with bronchial asthma was done.Results Compared with the control group,the levels of white blood cell(WBC),eosinophil(EO),monocyte(MO),neutrophil(NE),platelet(PLT),mean platelet volume (MPV) and platelet hematocrit(PCT) in the case group were significantly increased,and the levels of basophil(BA) and mean corpuscular volume(MCV) were significantly decreased(P<0.05).The areas under receiver operating characteristic curve(AUC) of WBC,BA,MO,NE,PLT,PCT,MPV,MCV in the children with bronchial asthma were statistically significant(P<0.05).The AUC of MPV,PCT and BA were 0.871,0.830 and 0.756,respectively.At the Cut-off value,the diagnostic sensitivity of MPV was the highest(0.963) and the diagnostic specificity of PCT was the highest (0.881).Conclusion The blood cell parameters in children with bronchial asthma show significant abnormalities in granulocytes ,MO and PLT.Among them,MPV and PCT have higher diagnostic value and may be used as potential indicators of asthma,while the reliability needs further study.
