摘要
目的探讨血清微小RNA-145(microRNA-145,miR-145)、miR-29与miR-217检测在膀胱癌中的临床价值。方法选取2016年2月至2018年5月在我院经穿刺或术中病理切片确诊的膀胱癌患者80例,同期收取80例膀胱炎患者和80例健康体检者。应用实时荧光定量PCR(quantitative real-timePCR,RT-PCR)仪对血清miR-145、miR-29与miR-217的相对表达量进行检测。结果膀胱癌患者血清miR-145、miR-29和miR-217表达水平(检测结果经负对数转换后呈正态分布)显著低于膀胱炎患者和健康对照者(P<0.05),而膀胱炎患者和健康对照者间血清miR-145、miR-29和miR-217水平差异均无统计学意义(P>0.05)。血清miR-145、miR-29与miR-217表达水平与膀胱癌淋巴结转移及TNM分级呈线性关联(P<0.05)。ROC曲线分析显示,血清miR-145、miR-29和miR-217在区分膀胱癌与膀胱炎患者和健康对照的灵敏度/特异性分别为81.3%/82.9%、82.7%/89.7%和77.2%/83.3%;而联合三者后在区分膀胱癌与膀胱炎患者和健康对照的灵敏度显著提高,达到91.7%。二元Logistic回归分析显示,在校正了年龄、性别、BMI、吸烟、饮酒后,血清低miR-145、低miR-29和低miR-217是膀胱癌的独立危险因素。结论血清低miR-145、低miR-29和低miR-217水平对膀胱癌的诊断以及在评估膀胱癌患者发生肿瘤远处转移中均具有重要的临床意义。
Objective To investigate the clinical value of microRNA-145(miR-145),miR-29 and miR-217 in bladder cancer.Methods From February,2016 to May,2018,80 patients with bladder cancer who were confirmed by biopsy or intraoperative pathology were selected for the study.During the same period,80 patients with cystitis and 80 healthy people were also enrolled.Real-time fluorescence quantitative PCR( RT-PCR )was then used to detect the relative expression of serum miR-145,miR-29 and miR-217 in all samples.Results The levels of serum miR-145,miR-29 and miR-217 in patients with bladder cancer were significantly lower than those in patients with cystitis and healthy controls( P <0.05).There were no significant differences in the levels of serum miR-145,miR-29 and miR-217 between patients with cystitis and healthy controls( P >0.05).Serum levels of miR-145,miR-29 and miR-217 in patients with bladder cancer had a linear correlation with lymph node metastasis and TNM grade( P <0.05).ROC curve analysis showed that the sensitivity / specificity of serum miR-145,miR-29 and miR-217 in distinguishing bladder cancer from cystitis patients and healthy controls were 81.3%/82.9%,82.7%/89.7%and 77.2%/83.3%,respectively;and the sensitivity(91.7%)of combining the three markers was significantly improved in distinguishing bladder cancer from cystitis patients and healthy controls.The binary logistic regression analysis showed that after adjusting for age,sex,BMI,smoking and drinking,serum levels of low miR-145,low miR-29 and low miR-217 were independent risk factors for bladder cancer.Conclusion Serum levels of low miR-145,low miR-29 and low miR-217 have important clinical significance in the diagnosis of bladder cancer and in the evaluation of distant metastasis of bladder cancer.
