摘要
目的应用时间-放射性曲线(TAC)及主动脉弓峰半高宽(FWHM)评价三通管注射法和头皮针注射法在单光子发射计算机断层显像脑血流灌注显像中的"弹丸"质量。方法将2016年6月至2017年2月间在中国医科大学附属第一医院核医学科行99mTc-ECD脑血流灌注显像的80例患者随机分成2组,每组40例,分别采用三通管注射法(A组)与头皮针注射法(B组)进行"弹丸"注射。2组患者静脉穿刺部位相同,且穿刺均成功。测量2组TAC的FWHM,结合肉眼比较2组TAC曲线形态,比较2组"弹丸"质量的差异以及"弹丸"质量良好的FWHM的差异。结果A组"弹丸"质量良好的患者37例(92.50%),"弹丸"质量欠佳的患者2例(5.00%),"弹丸"质量不合格的患者1例(2.50%);B组"弹丸"质量良好的患者39例(97.50%),"弹丸"质量欠佳的患者1例(2.50%),"弹丸"质量不合格的患者0例(0.00%);2组比较",弹丸"质量的差异无统计学意义(χ^2=1.386,P>0.05)。肉眼比较,2组"弹丸"质量良好的TAC曲线形态无明显差别。比较2组中"弹丸"质量良好的FWHM,差异无统计学意义(t=0.806,P>0.05)。结论三通管注射法与头皮针注射法均能成功完成脑血流灌注显像的"弹丸"注射,并均可保证良好的"弹丸"注射质量。
Objective To evaluate the quality of bolus injection in single-photon emission computed tomography cerebral blood flow perfusion imaging by using full width at half maximum( FWHM) analysis of the aortic arch in three-way injection and scalp injection. Methods Eighty patients who underwent technetium-99m-ethyl cysteinate dimer( 99mTc-ECD) cerebral perfusion imaging at our hospital from June 2016 to February 2017,in whom successful venipuncture at the same vein puncture site was done through three-way injection (group A,n = 40) or scalp injection( group B,n = 40),were selected.The FWHM of the time-activity curve( TAC) was measured,and the shape of the TAC was compared through naked-eye observation.The difference in the quality of bolus injection and the FWHM of bolus injections with good quality were compared.Results With respect to the quality of bolus injection,37 cases( 92.50%) had good quality, 2 cases( 5.00%) had poor quality,and 1 case( 2.50%) had an unqualified quality in group A.In group B,39 cases( 97.50%) had good quality,1 case( 2.50%) had poor quality,and no case had an unqualified quality of bolus injection.There was no significant difference in the quality of bolus injection between the two groups(χ^2=1.386,P > 0.05).No significant between-group differences were observed in the shape of the TAC.There was no significant difference in the FWHM of bolus injections with good quality between the two groups( t = 0.806, P > 0.05).Conclusion Both three-way injection and scalp injection can ensure the success and quality of bolus injection in 99mTc-ECD cerebral blood flow perfusion imaging.
