摘要
目的:探讨扩大性切除少突胶质细胞瘤的临床安全性及远期疗效。方法:回顾性分析2013年10月至2018年10月收治的少突胶质细胞瘤病例的临床资料和影像资料。结果:共22例。位于非功能区16例,均行扩大性切除,无围手术期死亡病例,围手术期重残2例(12.5%),随访显示失访1例,复发并死亡4例(25%),重残生存1例,生活自理10例(62.5%)。位于功能区6例,根据T2像沿肿瘤-脑组织边界切除,不全切除2例(33.3%),围手术期死亡1例(16.7%),围手术期重残2例(33.3%),随访显示不全切除的2例病例复发增大(40%),重残3例(50%),生活自理2例(33.3%)。结论:对于少突胶质细胞瘤应当追求在保证病人神经功能的前提下达到全切除,对于非功能区者,推荐扩大性切除,位于功能区者的手术治疗不能让人满意。
Objective:To investigate the clinical safety and long-term efficacy of extended resection of oligodendroglioma.Methods:The clinical and imaging data of oligodendroglioma patients admitted from October 2013 to October 2018 were retrospectively analyzed.Results:All are 22 cases. Tumors of sixteen cases were located in non-functional areas, all of them underwent extended resection. There were no perioperative deaths. Two cases(12.5%) were severely disabled during the perioperative period.Follow-up showed that 1 case was lost, 4 cases(25%) recurred and died, 1 case survived with severe disability, and 10 cases(62.5%) took care of themselves.Tumors of six cases were located in functional area. According to T2 weight image, 4 cases underwent total resection and the other 2 cases(33.3%) underwent partial resection, 1 case(16.7%) died during the perioperative period, 2 cases(33.3%) were severely disabled during the perioperative period. Follow-up showed that the recurrence of 2 cases(40%) increased after incomplete resection, 3 cases(50%) were severely disabled and 2 cases(33.3%) were self-care.Conclusion:For oligodendroglioma, total resection should be pursued on the premise of guaranteeing the neurological function of patients. For the tumors of non-functional areas, extended resection is recommended. Extended surgical treatment for those lesions located in functional areas isnot a good choice.
