摘要
目的脑血管造影行股动脉穿刺后,采用旋钮式动脉压迫器对股动脉穿刺点进行压迫止血,评价其止血效果及相关并发症的发生情况,并对患者的感受进行舒适度评分。方法选取2017年1月~2018年8月行脑血管造影的患者120例,造影完毕后随机分为研究组和对照组,其中研究组60例患者采用旋钮式动脉压迫器进行压迫止血,根据患者血压情况压迫6~8h后拆除压迫器,对照组60例患者采用传统人工压迫的方式进行股动脉穿刺点止血,弹力绷带八字交叉进行加压包扎,24h后拆除绷带。结果脑血管造影结束后,采用动脉压迫器组的止血操作时间显著小于对照组,两组比较,差异有统计学意义(P<0.05);动脉压迫器组右下肢制动时间显著少于人工压迫组(P<0.05),可以根据患者右下肢的感受调整压迫器的深度,患者术后舒适程度动脉压迫器组优于人工压迫组(P<0.05);压迫器用力点精准,力度可调,便于观察,术后并发症动脉压迫器组少于人工压迫组(P<0.05)。结论动脉压迫器可以更好地对股动脉穿刺点进行压迫止血,能够人性化的调整压迫深度,减少右下肢制动时间,降低并发症的发生率,缓解患者的术后焦虑状态,有助于提高患者的术后舒适度和对脑血管造影检查的依从性,可以在介入导管室推广使用。
Objective After the femoral artery puncture was performed by cerebral angiography,the femoral artery puncture site was subjected to compression and hemostasis by using an arterial compressor.Its hemostasis effect and the occurrence of related complications were evaluated,and the patients were graded for comfort.Methods Totally 120 patients were selected for cerebral angiography from Jan.2017 to Aug.2018.After the angiography was completed,the patients were randomly divided into the study group and the control group.Totally 60 patients in the study group were treated with arterial compressors for hemostasis,and the compressors were removed after 6-8 hours according to the patient′s blood pressure.Totally 60 patients in the control group received femoral artery puncture point hemostasis through traditional manual compression,and received compression bandage with spline cross,and the bandage was removed 24 hours later.Results After the cerebral angiography,the hemostatic operation time of the arterial compressors group was significantly shorter than that of the control group,and there was a significant difference between the two groups ( P <0.05).The constraining time of right lower limb of the arterial compressor group was significantly less than that of the manual compression group ( P <0.05).The depth of the compression device could be adjusted according to the patient′s feeling of right lower limb.The postoperative comfort level of the arterial compressor group was better than that of the manual compression group ( P <0.05).The pressure points of the compressors were accurate and convenient for observation,and the postoperative complication of the arterial compressors group was less than that of the manual compression group ( P <0.05).Conclusion Arterial compressors can better perform compression hemostasis on the femoral artery puncture site,which can humanize to adjust the compression depth,reduce the constraining time of right lower limb,reduce the incidence of complications,alleviate the postoperative anxiety of the patient,improve the postoperative comfort of the patient and comply with the cerebral angiography examination.This equipment can be popularized in the interventional catheterization room.
