摘要
目的讨论经鼻双水平正压通气(DuoPAP)联合气管插管-肺表面活性物质-拔管(INSURE)技术及肺表面活性物质(PS)治疗早产儿呼吸窘迫综合征(RDS)的临床效果。方法选择我院早产儿RDS患儿60例,采用数字表法分为两组,每组30例。观察组采用DuoPAP+INSURE技术+PS治疗;对照组采用经鼻无创正压通气(nCPAP)+INSURE技术+PS治疗。比较两组临床疗效、正压通气时间、氧疗时间、pH值、动脉血氧饱和度(SaO2)、动脉氧分压(PaO2)、动脉二氧化碳分压(PaCO2)。结果两组疗效比较,P<0.05;两组正压通气时间、氧疗时间比较,P均<0.05;两组治疗前,pH值、SaO2、PaO2、PaCO2比较,P均>0.05;两组治疗后,pH值、SaO2、PaO2、PaCO2比较,P均<0.05。结论DuoPAP+INSURE技术+PS治疗早产儿RDS,疗效确切,可有效缩短正压通气时间及氧疗时间,同时可明显改善患儿pH值、SaO2、PaO2、PaCO2,值得临床推广。
Objective To discuss the clinical application of transnasal bi-level positive airway pressure (DuoPAP) combined with tracheal intubation-pulmonary surfactant-extraction (INSURE) technique and pulmonary surfactant (PS) in the treatment of respiratory distress syndrome (RDS) in preterm infants.effect.Methods Sixty children with premature infants with RDS in our hospital were enrolled in the study.They were divided into two groups by digital table method,30 cases in each group.The observation group was treated with DuoPAP+InSurE technology+PS;the control group was treated with nasal noninvasive positive pressure ventilation (nCPAP)+ InSurE technology + PS.The clinical efficacy,positive pressure ventilation time,oxygen therapy time,pH value,SaO2,PaO2,PaCO2 were compared between the two groups.Results The effective rate of treatment was compared between the two groups (P<0.05).The positive pressure ventilation time and oxygen therapy time were compared between the two groups (P<0.05).Before treatment,the pH value,SaO2, PaO2,PaCO2 were compared,P>0.05.After treatment,the pH value,SaO2,PaO2,PaCO2 comparison,P<0.05.Conclusion DuoPAP+InSurE technology + PS treatment of premature infant RDS,the curative effect is exact,can effectively shorten the positive pressure ventilation time and oxygen therapy time,and can significantly improve the disease the PH value, SaO2,PaO2,PaCO2.
