腺样体肥大伴分泌性中耳炎患儿的免疫功能分析 被引量:12

摘要 目的通过比较腺样体肥大伴分泌性中耳炎(SOM)患儿及单纯腺样体肥大患儿腺样体组织中T淋巴细胞及炎症细胞因子的表达情况,探讨T淋巴细胞及炎症细胞因子与SOM发病的相关性。方法选取2015年3月至2018年2月期间舟山市妇幼保健院和舟山市定海区中心医院收治的单纯腺样体肥大患儿65例与腺样体肥大伴SOM患儿70例,采用免疫组化法分别检测两组患儿腺样体组织中CD3+、CD4+、CD8+T细胞的阳性表达量及CD4+T/CD8+T值,采用ELISA法分别检测两组患儿腺样体组织中炎症细胞因子肿瘤坏死因子(TNF-α)、白介素(IL)-2、IL-6的含量。结果 CD3+、CD4+、CD8+T细胞在观察组和对照组患儿的腺样体组织中均有表达,表达多集中于细胞膜或包浆中,显色为棕黄色。观察组患儿腺样体组织中CD3+、CD4+、CD8+T细胞的阳性表达量、CD4+T/CD8+T值及TNF-α、IL-2、IL-6的含量均显著高于对照组,组间比较差异均有统计学意义(t值分别为5.372、6.258、6.613、4.302、7.281、8.035、7.148,均P<0.05)。结论腺样体肥大伴SOM患儿腺样体局部T淋巴细胞及炎症细胞因子的表达均显著上升,可能诱导腺样体局部免疫功能失调,进而诱发分泌性中耳炎反复发作。 Objective To explore the correlation between T lymphocytes and inflammatory cytokines and the pathogenesis of secretory otitis media(SOM) by comparing the expressions of T lymphocytes and inflammatory cytokines in adenoid tissues of children with adenoid hypertrophy complicated with SOM and children with adenoid hypertrophy. Methods Sixty-five children with adenoid hypertrophy and 70 children with adenoid hypertrophy complicated with SOM were enrolled in Zhoushan Maternal and Child Health Care Hospital and Zhoushan Dinghai District Central Hospital from March 2015 to February 2018. Immunohistochemistry was used to detect the positive expressions of CD3+, CD4+ and CD8+ T cells and the ratio of CD4+T/CD8+T in adenoid tissues of two groups. ELISA was used to detect the levels of inflammatory cytokines TNF-α, IL-2 and IL-6 in adenoid tissues of two groups. Results CD3+, CD4+, and CD8+ T cells were expressed in the adenoid tissues of both groups, and the expression was mostly concentrated in the cell membrane or the cytoplasm, and the coloration was brownish yellow. The positive expression levels of CD3+, CD4+, CD8+ T cells, CD4+T/CD8+T and the levels of TNF-α, IL-2 and IL-6 in the adenoid tissues of the observation group were significantly higher than those in the control group. The differences between two groups were statistically significant(t value was 5.372, 6.258, 6.613, 4.302, 7.281, 8.035 and 7.148, respectively, all P<0.05). Conclusion The expressions of T lymphocytes and inflammatory cytokines in adenoid tissues of children with adenoid hypertrophy complicated with SOM increase significantly, which may induce local adenoid tissues immune dysfunction and then induce repeated attacks of SOM.
