DWI序列及ADC值诊断脊柱椎体良恶性压缩骨折的价值 被引量:3

The Value of DWI Sequence and ADC Value in the Diagnosis of Benign and Malignant Compression Fractures of the Spine

摘要 目的:探讨磁共振弥散加权成像(DWI)及表观扩散系数(ADC)值在脊柱椎体良恶性压缩骨折中的应用价值。方法:选择2016年11月—2018年8月我院收治的104例脊柱椎体良恶性压缩骨折患者为研究对象,所有患者均经组织病理学检查确诊,根据病理学诊断结果分为良性组(42例)和恶性组(62例),均行DWI和常规磁共振成像(MRI)扫描,对比分析两种检查方式在脊柱椎体良恶性压缩骨折中的诊断效能。结果:常规MRI检查在脊柱椎体良恶性压缩骨折中的诊断灵敏度71.4%、特异度61.3%和准确度65.4%,DWI及ADC值的诊断灵敏度85.7%、特异度90.3%和准确度88.5%,二者比较差异有统计学意义( P <0.05);良性组和恶性组DWI各项信号所占比例比较,差异无统计学意义( P >0.05),但良性组ADC值显著高于恶性组,差异有统计学意义( P <0.05)。结论: DWI及ADC值可以更好地鉴别诊断脊柱椎体良恶性压缩骨折的病理类型,其诊断价值优于常规MRI检查。 Objective: To investigate the value of diffusion-weighted imaging (DWI) and apparent diffusion coefficient (ADC) in the treatment of benign and malignant compression fractures of the vertebral body. Methods: A total of 104 patients with benign and malignant vertebral fractures of the spine were enrolled in our hospital from November 2016 to August 2018. All patients were diagnosed by histopathological examination.Divided into benign group (42 cases) and malignant group (62 cases), both magnetic resonance diffusion weighted imaging (DWI) and conventional magnetic resonance imaging (MRI) scan, comparative analysis of two examination methods in the spinal vertebral body and benign compression fracture Diagnostic efficacy. Results: The diagnostic sensitivity of routine MRI in the diagnosis of benign and malignant vertebral fractures of the spine was 71.4%,specificity 61.3% and accuracy 65.4%,diagnostic sensitivity 85.7%,specificity 90.3% and accuracy 88.5% of DWI and ADC values,the difference was statistically significant ( P <0.05);the proportion of DWI signals in benign group and malignant group was not statistically significant ( P >0.05),but benign group ADC value was significantly higher than the malignant group,the difference was statistically significant ( P <0.05). Conclusion: Magnetic resonance diffusion-weighted imaging (DWI) and apparent diffusion coefficient (ADC) values can better differentiate the pathological types of benign and malignant compression fractures of the vertebral body.The diagnostic value is better than conventional MRI.
