摘要
目的分析某院眼科专业各病区的DRGs相关指标,发现优势与不足,为该专业的临床重点专科建设和精细化管理提供参考.方法结合CN-DRGs分组方式,利用MDCC的DRGs组数、病例组合指数(CMI)、费用消耗指数等DRGs指标,以及药占比、耗材比、抗菌药物使用率等管理指标,利用加权Topsis法对2018年上半年眼科5个病区住院医疗服务绩效进行评价.结果加权Topsis法评价结果显示,眼科A病区服务能力最强,服务效率最优,综合排名第1,但其在目标管理上仅居第3位;B病区在服务能力、服务效率及综合排名中均位列第3,但目标管理居最后;C病区发展较为均衡,在各维度排名中均位列第2,其综合排名也位居第2;D病区在服务能力和综合排名中位列第4,在服务效率排名中位列第5,但在目标管理上排名第1;E病区在服务能力和综合排名中位列第5,在服务效率和目标管理排名中位列第4.结论利用DRGs和Topsis法对眼科住院医疗服务绩效进行综合评价,可以客观评价医疗服务能力、服务效率及目标管理情况,为该专业加强学科建设、绩效考核和精细化管理提供数据支持.
Objective To analyze the DRGs indicators of ophthalmology to find advantages and disadvantages and provide suggestions for the development of clinical key specialist construction and delicacy management. Methods The "CN - DRGs" mode was used to obtained the DRGs data. Evaluation indicators contained DRGs indicators (the amount of DRGs group, case mix index, cost consumption index, etc.) and target management indicators (percentage of drug expenditure, percentage of medical consumables expenditure, percentage of antimicrobial application, etc.).Weighted Topsis evaluation method was used to evaluate the inpatient hospitalization services performance in 5 Ophthalmology Department in the first half of 2018. Results The results of weighted Topsis evaluation showed that ward A had the strongest medical service capacity and the best service efficiency with overall first rank and target management of third rank. Ward B ranked third in service capacity, service efficiency as well as overall ranking, but it ranked 5th in target management. Ward C was relatively balanced, ranking second in all latitudes as well as the overall ranking. Ward D ranked 4th in service capacity and overall ranking. It ranked 5th in service efficiency but ranked top 1 in target management. Ward E ranked 5th in service capacity and overall ranking. It ranked 4th in service efficiency and target management. Conclusion The DRGs and lopsis methods were used to comprehensively evaluate the performance of ophthalmic inpatient medical services, which can objectively evaluate their medical service capability, service efficiency and target management, and provide data support for strengthening discipline construction, performance assessment and fine management.
