血小板参数、C反应蛋白和脂蛋白a联合检测在急性脑梗死预测中的应用价值研究 被引量:4

摘要 目的分析血小板参数、C反应蛋白(CRP)和脂蛋白a[LP(a)]联合检测在急性脑梗死预测中的应用价值。方法 88例急性脑梗死患者,参照神经功能缺损评估标准分为轻度组(30例)、中度组(30例)、重度组(28例),另选同期进行健康体检的50例健康人作为对照组。观察对比四组血小板参数、CRP及LP(a)水平。结果轻、中、重度组的血小板(PLT)、血小板压积(PCT)水平均低于对照组,血小板分布宽度(PDW)及平均血小板体积(MPV)水平均高于对照组,差异具有统计学意义(P<0.05);重度组的PLT、PCT水平均低于轻、中度组, PDW、MPV水平均高于轻、中度组,差异具有统计学意义(P<0.05);中度组的PLT、PCT水平均低于轻度组, PDW、MPV水平均高于轻度组,差异具有统计学意义(P<0.05)。轻、中、重度组及对照组的LP(a)水平分别为(165.25±2.36)、(196.35±2.34)、(225.34±5.26)、(105.35±5.31)mg/dl, CRP水平分别为(7.01±1.02)、(16.25±2.05)、(22.70±1.04)、(1.25±0.26)mg/L。轻、中、重度组的LP(a)及CRP水平均明显高于对照组,差异具有统计学意义(P<0.05);重度组CRP及LP(a)水平明显高于轻、中度组,中度组CRP与LP(a)水平明显高于轻度组,差异具有统计学意义(P<0.05)。结论急性脑梗死的早期诊断对降低病死率具有重要意义,而血小板参数、CRP及LP(a)水平在疾病发生前后、不同病情程度中均会呈现出显著差异,可作为急性脑梗死预测及病情评估的关键性指标。
