摘要
目的探讨螺旋CT肺动脉造影联合低剂量下肢(CT venography,CTV)在下肢深静脉血栓(Deep venous thrombosis,DVT)与肺动脉栓塞(Pulmonary embolism,PE)的相关性。方法2017年7月-2019年1月,行下肢深静脉多普勒超声(Ultrasound,US)疑诊DVT患者178例,记录左下肢DVT、右下肢DVT及双下肢DVT同时出现PE的例数,分析相关性。结果 CTPA联合低剂量CTV检查发现DVT病变165处,其中左下肢104处,发生率63.03%(104/165),右下肢61处,发生率36.97%(61/165),左下肢DVT发生率明显高于右下肢,差异具有统计学意义(χ~2=24.567,P <0.05)。DVT患者合并PE的总发生率为47.88%(79/165),左下肢DVT合并PE发生率59.62%(62/104),右下肢DVT合并PE发生率27.87%(17/61),差异具有统计学意义(χ~2=9.458,P <0.05),PE的发生于DVT间具有相关性且呈正相关(r=0.793,P=0.028)。结论CTPA联合低剂量CTV检查对DVT及PE的显示效果较好,同时PE与DVT发生间具有相关性。
Objective Purpose To investigate the correlation between spiral CT pulmonary angiography combined with low-dose lower extremity CTV in deep venous thrombosis(DVT) and pulmonary embolism(PE). Methods From July 2017 to January 2019, 178 patients with suspected DVT who underwent deep vein Doppler ultrasound(US) were enrolled. The number of PEs in the left lower extremity DVT, right lower extremity DVT and double lower extremity DVT were recorded. Correlation. Results CTPA combined with low-dose CTV examination found 165 lesions of DVT, including 104 in the left lower limb, the incidence was 63.03%(104/165), 61 in the right lower limb, the incidence was 36.97%(61/165), and the incidence of DVT in the left lower limb was significantly higher. In the right lower extremity,the difference was statistically significant(χ~2 =2 4.567, P < 0.05). The total incidence of PE in patients with DVT was 47.88%(79/165), the incidence of DVT in the left lower extremity was 59.62%(62/104), and the incidence of DVT in the right lower extremity was 27.87%(17/61). The difference was statistically significant. The significance of learning(χ~2 = 9.458, P <0.05), PE had a correlation between DVT and was positively correlated(r = 0.793, P = 0.028). Conclusion CTPA combined with low-dose CTV examination shows better results for DVT and PE, and there is a correlation between PE and DVT.
