摘要
目的分析全身麻醉与椎管内麻醉应用于前置胎盘剖宫产术中对产妇与胎儿的影响。方法选取该院2016年4月—2019年3月收治的68例前置胎盘产妇为研究对象,以麻醉方式为依据,将其分为两组,甲组行全身麻醉,乙组行椎管内麻醉,对比两组产妇术中升压药应用率、输液量、术中失血量、手术时间、新生儿Apgar评分以及断脐时间。结果甲组术中输液量、术中出血量多于对照组,手术时间长于对照组,差异有统计学意义(t=10.2598、8.5558、11.0267,P<0.05);甲组升压药物应用率为32.35%(11/34),高于乙组的5.88%(2/34),差异有统计学意义(χ^2=8.5528,P<0.05);对比两组新生儿Apgar评分、断脐时间,均差异无统计学意义(P>0.05)。结论对于出血量大、严重前置胎盘产妇,剖宫产术中可选择全身麻醉,其他类型产妇可以实际情况为依据,对麻醉方式进行灵活选择。
Objective To analyze the effects of general anesthesia and spinal anesthesia on maternal and fetal outcomes during cesarean section of placenta previa. Methods 68 cases of placenta previa were treated in our hospital from April 2016 to March 2019. According to the anesthesia method, they were divided into two groups. Group A underwent general anesthesia and group B underwent spinal canal anesthesia, to compare the application rate, infusion volume, intraoperative blood loss, operation time, neonatal Apgar score, and umbilical cord time of the two groups. Results In the group A, the intra-operative fluid volume and intraoperative blood loss were more than the control group. The operation time was longer than that of the control group. The difference was statistically significant (t=10.259 8, 8.555 8, 11.026 7, P<0.05);the application rate of booster drug of 32.35%(11/34), which was higher than 5.88%(2/34) of group B. The difference was statistically significant (χ^2=8.552 8, P<0.05). The differences between Apgar score and umbilical cord time were compared between the two groups. There was no statistical significance (P>0.05). Conclusion For patients with large bleeding volume and severe placenta previa, general anesthesia can be selected during cesarean section. Other types of women can be based on actual conditions and flexible choice of anesthesia.
