体外膜肺氧合联合主动脉球囊反搏救治1例重症暴发性心肌炎 被引量:8

摘要 探讨体外膜肺氧合救治重症暴发性心肌炎的疗效。患者,男性,53岁,因“突发意识障碍6h”入院,急诊行主动脉内球囊反搏(Intra-aortic balloon pump,IABP)下的冠状动脉造影,左主干、左前降支及左回旋支及右冠未见明显狭窄,远端血流TIMI3级,超声心动图示左室壁运动弥漫性减低,左心室射血分数23%,下腔静脉吸气末宽度为2.92cm。急诊床边安装V-AECMO辅助治疗,第5天超声心动图示左心室射血分数56%,平均动脉压75mmHg,中心静脉氧饱和度78%,固逐步减低ECMO流量至1.0L/min,撤离ECMO并拔除股动脉、股静脉导管。第6天拔除气管插管,第12天转回心内科,第34天出院,2个月、4个月电话随访,超声心动图示射血分数分别为58%、62%。重症暴发性心肌炎发病率,但死亡率高,ECMO作为一项新的心肺支持技术,能稳定患者血流动力学,协助度过急性期,提高抢救成功率。其应用于FM所致的难治性心源性休克患者疗效确切,临床上应该加以推广。
