摘要
目的研究老年2型糖尿病冠状动脉介入术(PCI)后患者糖化血红蛋白(HbA1c)水平与心功能的关系.方法回顾性分析2017年1月至2017年12月郑州大学第一附属医院收治的198例老年2型糖尿病PCI术后患者的临床资料,198例患者按HbA1c分为两组:I组(HbA1c<7%)52例,Ⅱ组(HbA1c≥7%)146例.比较两组年龄、糖尿病病程、收缩压、舒张压、空腹血糖、总胆固醇、三酰甘油、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、肌钙蛋白T、植入支架个数、B型利钠肽前体(BNP)、左室射血分数(LVEF),采用logistic回归分析老年2型糖尿病PCI术后患者心功能异常的影响因素.结果两组患者的年龄、糖尿病病程、空腹血糖、BNP、LVEF比较差异均有统计学意义(P均<0.05);HbA1c≥7%是PCI术后患者心功能异常的危险因素(OR=5.717,95%CI1.549~21.101).结论对于老年2型糖尿病PCI术后患者,HbA1c≥7%是心功能异常的危险因素.
Objective To study the relationship between glycosylated hemoglobin interventio(HbA1c)and cardiac function in elderly patients with type 2 diabetes after percutaneous coronary intervention(PCI).Methods The data of 198 elderly patients with type 2 diabetes mellitus after PCI in the First Affiliated Hospital of Zhengzhou University from January 2017 to December 2017 were retrospectively analyzed,and they were grouped,with 52 patients in group I(HbA1c<7%)and 146 patients in groupⅡ(HbA1c≥7%).The age,duration of diabetes,systolic blood pressure,diastolic blood pressure,fasting plasma glucose(FPG),total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),troponin T,number of implanted stents,B-type natriuretic peptide precursor(BNP),left ventricular ejection fraction(LVEF)were compared between the two groups.Influential factors of cardiac dysfunction in elderly patients with type 2 diabetes after PCI were analyzed by logistic regression.Results There were significant differences in age,duration of diabetes,FPG,BNP and LVEF between the two groups(P<0.05).HbA1c≥7%was a risk factor for cardiac dysfunction in patients after PCI(OR=5.717,95%CI 1.549-21.101).Conclusions In elderly patients with type 2 diabetes mellitus after PCI,HbA1c≥7%is a risk factor for cardiac dysfunction.
