池州地区HPV感染的基因型分布特点和宫颈病变情况分析 被引量:13

摘要 目的:探讨池州地区人乳头瘤病毒(HPV)感染基因型分布特点及与宫颈病变发生的相关性。方法:对2017年12月至2018年12月在池州市人民医院就诊的683例宫颈病变患者行阴道镜检查及病理活检,对其HPV感染状况、基因型分布与宫颈病变间的关系进行分析。结果: 683例患者,其中558例存在HPV感染。高危型最常见型别为:HPV16型,其次为HPV52型,HPV58型,HPV18型,HPV33型。低危型为:HPV81。宫颈低级别鳞状上皮内病变(LSIL)中最常见的基因型为:HPV52型,其次为HPV16型,HPV18型,HPV58型,HPV33型。宫颈高级别鳞状上皮内病变(HSIL)中最常见的基因型为:HPV16型,其次为HPV52型,HPV58型。癌阳性中感染频数最高的基因型为HPV16型,其次为HPV52型,HPV18 型,HPV58型。各类宫颈病变的单一感染和多重感染的构成比的差异无统计学意义(χ 2=6.554, P =0.088)。LSIL峰值为41~50岁,HSIL峰值为31~40岁和41~50岁。结论:池州地区宫颈病变患者,常见HPV感染型别是HPV16和18型,本地区HPV52、58、33型感染也较高,应及时干预治疗,预防宫颈癌的发生。 Objective: To investigate the genotype distribution of HPV infections and the corresponding correlation with cervical lesions in Chizhou city. Methods: From December 2017 to December 2018,683 patients with cervical lesions were treated in the colposcope specialist clinic of Chizhou People's Hospital,including the vaginoscopy and pathological biopsy.The correlation of HPV infection,type distribution and cervical lesions was analyzed and discussed from these cases. Results: In the above mentioned 683 cases,558 cases of HPV infection had been found.The most common genotype with high-risk was HPV16,followed by HPV 52,HPV 58,HPV 18 and HPV 33,and HPV81 was the low-risk type.The most common genotype in LSIL was HPV52,followed by HPV16,HPV18,HPV58 and HPV33.The most common genotype in HSIL was HPV16,followed by HPV52 and HPV58.The genotype with the highest infection frequency was HPV16,followed by HPV52,HPV18 and HPV58.There was no significant difference in the composition ratio between single infection and multiple infection of various cervical lesions(χ 2=6.554, P =0.088).The peak age of LSIL was 41-50,and the peak ages of HSIL included two segments:31-40 and 41-50. Conclusion: Among the patients with cervical lesions in Chizhou,the common HPV infection types are HPV16 and 18.The HPV52,58 and 33 infections are also higher in this area.Therefore,timely intervention and treatment should be taken to prevent the occurrence of cervical cancer.
