体外冲击波治疗骨不连27例临床观察 被引量:4

摘要 目的观察体外冲击波对27例四肢骨不连患者的临床疗效。方法采用瑞士EMS体外冲击波对本院2017年7月至2018年5月收治的27例(男16例,女11例,年龄12~53岁)四肢骨折骨不连的患者进行治疗,设置高能体外冲击波治疗(1.0~3.5 bar/5 000次/部位),冲击频率为8~10 Hz,每5天1次。4次为1个疗程,共治疗2个疗程后,定期复查X线,观察骨折的临床愈合情况,并进行疗效评定。结果治疗后27例患者均获得6~12个月的随访。要求患者定期复查,并对X线结果进行详细记录。本次研究对象27例中,23例骨不连部位骨折完全愈合或有明显骨痂生成,4例随访时间内未见改善。患者的骨折平均愈合时间为治疗后19周,临床总有效率为85.19%,且治疗过程中及治疗后未见骨不连周围的神经病变、局部肿胀及淤血、内固定系统断裂等不良反应。结论体外冲击波(Swiss EMS)治疗骨不连是一种无创、安全的方法,操作简单,对患者经济负担小、治疗效果好。 Objective To observe the clinical effect of extracorporeal shock wave on 27 cases of nonunion. Methods From July 2017 to May 2018, 27 cases(16 males, 11 females, 12-53 years) of limb fracture nonunion in our hospital were treated by Swiss EMS extracorporeal shock wave. High-energy extracorporeal shock wave therapy(1.0-3.5 bar/5 000 times/site) was set up. The frequency of shock was 8 to 10 Hz, once every 5 days. As a course of treatment, the treatment lasted four times, X-ray examination was performed regularly and then observation of the fracture healing after two courses of treatment, evaluating the curative effect in the end. Results After treatment, the 27 patients were followed up for 6 to 12 months. Patients were asked to do regular review, and the X-ray inspection results were recorded in detail.Through the observation of 27 cases of this study, 23 patients had complete union or obvious callus formation at the nonunion site, the remaining 4 patients did not improve during the subsequent period. The average healing time of fracture was 19 weeks after treatment. The total effective rate was 85.19%. There were no adverse reactions such as neuropathy,local swelling, congestion and fracture of internal fixation system around nonunion during and after treatment. Conclusion Extracorporeal shock(Swiss EMS) wave treatment of nonunion is noninvasive, safe, simple to operate, less economic burden for patients, and effective.
