鼻咽癌病人同期放化疗个体化营养干预的效果评价 被引量:14

Evaluation of individualized nutrition intervention for nasopharyngeal carcinoma with concurrent radiotherapy and chemotherapy

摘要 [目的]探讨基于病人主观整体营养状况评估量表(PG‐SGA)营养评估的营养干预流程和个体化营养干预对改善鼻咽癌同期放化疗病人营养状况的效果。[方法]选取2017年12月-2018年6月在广西壮族自治区3所三级甲等医院放疗病区治疗的145例鼻咽癌病人,按随机数字表法分为对照组73例和观察组72例,对照组给予放疗科常规营养教育和饮食指导,干预组根据PG‐SGA评分结果、放射性口腔黏膜的反应程度、病人的年龄、体重、身高给予病人个体化的营养干预,指导病人实施同期放化疗常见毒副反应的预防与干预措施。比较两组干预效果。[结果]放疗15次、30次对照组PG‐SGA评分分别为(12.15±5.08)分、(20.31±6.01)分,观察组分别为(7.33±3.05)分、(12.33±5.28)分,两组比较差异均有统计学意义(P<0.05);放疗15次、30次两组无食欲、恶心、呕吐、疼痛、放射性口腔黏膜炎反应程度、血红蛋白、血清前清蛋白水平等比较差异有统计学意义(P<0.05)。[结论]放疗前评估病人的营养状况,及时给予病人科学合理的个体化营养干预和放化疗毒副反应的管理,可有效改善病人的营养状况,进而提高病人的机体免疫力及生活质量。 Objective:To explore the nutritional intervention process based on Patient-Generated Subjective Global Assessment(PG-SGA), and to evaluate the effect of individualized nutrition intervention on improving the nutritional status of patients with nasopharyngeal carcinoma undergoing concurrent radiotherapy and chemotherapy. Methods:A total of 145 patients with nasopharyngeal carcinoma treated in the radiotherapy area of three Tertiary Grade A hospitals in Guangxi Zhuang Autonomous Region from December 2017 to June 2018 were involved in this study. They were randomly divided into the control group(73 cases)and the observation group(72 cases). The control group received routine nutrition education and dietary guidance in the radiotherapy department. The observation group received individualized nutritional intervention according to the PG ‐SGA score,the reaction degree of the radioactive oral mucosa,patient′s age, weight,and height. The observation group was guided to implement the prevention and intervention of common toxic and side effects of concurrent radiotherapy and chemotherapy. The results of the intervention were compared between the two groups. Re su l t s:The PG ‐ SGA scores of the control group were(12. 15±5. 08)points in 15 times of radiotherapy,(20. 31±6. 01)points in 30 times of radiotherapy,and the scores of the observation group were(7. 33±3. 05)points and(12. 33±5. 28)points,respectively. The differences between the two groups were statistically significant(P<0. 05). There were all significant differences in no appetite,nausea,vomiting, pain,the degree of radioactive oral cavity mucositis reaction,hemoglobin,and serum pre-albumin levels(P<0. 05)between 15 times and 30 times of the radiotherapy. Conclusions:Evaluating the nutritional status of patients before radiotherapy and giving scientific and rational individualized nutrition intervention and management of toxic and side effects of radiotherapy and chemotherapy timely could effectively improve the nutritional status of patients,and improve their immunity and quality of life further.
