信息化定量管理结合进阶式教学在临床病理科住院医师规范化培训中应用的探索 被引量:15

摘要 目的探讨信息化定量管理结合进阶式教学模式在临床病理住院医师规范化培训(住培)管理中的作用。方法自2014年开始,完善病理信息管理系统功能,对临床病理科专业住院医师进行信息化定量考核管理。培训进行一年半以后,以定量考核结果为依据,安排进阶式分层“一对一”教学指导,直到培训结束。选取完成住培的学员(研究组)和科室未经过住培(对照组)的低年资主治医师各5名,通过信息系统分别提取两组人员日常工作中同工作年限时期的疑难病例上交率及上交疑难病例初诊符合率,进行统计学分析。结果研究组初诊符合率在前2年内以平均每季度3%~5%的幅度稳步提高,2年后符合率平均达到约90%。培训结束的学员在临床签发报告的工作中与对照组相比,疑难病例上交率降低3.50%(P=0.303),上交疑难病例初诊符合率升高8.85%(P=0.038)。结论信息化定量管理结合进阶式教学模式可有效提高临床病理科住院医师的岗位胜任能力,根据信息系统的测算数据,病理科可按实际工作情况制订合适的培训计划和分阶段目标及要求,并根据学员的考核表现进行动态指导和管理。 Objective The aim of this study was to explore the role of quantitative information management combined with advanced teaching mode in the standardized residency training of the pathology department.Methods Since 2014,the functions of the pathological information management system have been improved and the quantitative assessment and management of pathology residents have been established.After one and a half years of training,according to the results of quantitative assessment,the advanced layered“one-to-one”teaching guidance will be arranged until the end of the three-year training period.Data from five pathology residents who completed the standardized training and other five without regular training were collected and compared.The information system was used to extract the applications and diagnostic accuracy rates of difficult cases consultation in the daily work of two groups for statistical analysis.Results The accuracy rates of the newly-trained trainees increased steadily in the first two years with an average of 3%~5%per quarter.After two years,the diagnostic accuracy rates of commonly and frequently encountered diseases reached average about 90%.At the end of the training,compared with untrained doctors,the rates of application for difficult cases consultation decreased by 3.50%(P=0.303)in the trained group,and the accuracy rates for difficult cases diagnosis increased by 8.85%(P=0.038).Conclusion Quantitative information management combined with advanced teaching mode can effectively improve the post competence of clinical pathology doctors.According to the measurable data of the information system,the pathology department can be guided to make appropriate training plans according to their actual work conditions.Furthermore,the guidance and dynamic management should be performed based on the assessment results of the participants.
