不同程度氟斑牙患者拔牙矫治对支抗的影响 被引量:1

Effect of tooth extraction on anchorage in patients with different degrees of dental fluorosis

摘要 目的探讨不同严重程度的氟斑牙在正畸治疗过程中对成人安氏Ⅰ类拔牙患者的上颌磨牙和切牙位置的影响。方法选取2010年7月至2018年7月天津市口腔医院正畸科诊治的正畸患者60例,根据Dean法氟斑牙诊断标准分为重度组、中度组、轻度组和对照组,每组15例。应用Dolphin测量软件筛选符合标准的患者,对患者治疗前后的X线头颅侧位片进行测量和比较分析。测量指标:(1)上颌中切牙长轴与上颌腭平面(PP)的交角;(2)上颌第一磨牙长轴与上颌PP的交角(U6toPP);(3)上颌中切牙切缘至垂直PP的平面(PTV)的距离;(4)上颌中切牙根尖点至PTV的距离;(5)上颌第一磨牙近中最突点至PTV的距离(U6M-Hor);(6)上颌第一磨牙近中根最低点至PTV的距离(U6A-Hor)。结果轻度组患者6项测量指标与对照组比较,差异均无统计学意义(P>0.05);中度组患者U6toPP、U6M-Hor、U6A-Hor与对照组比较,差异均有统计学意义(P<0.05);重度组患者6项测量指标与对照组比较,差异均有统计学意义(P<0.05)。轻、中度组患者6项测量指标与重度组比较,轻度组患者U6toPP、U6M-Hor、U6A-Hor与中度组比较,差异均有统计学意义(P<0.05)。结论不同程度的氟斑牙对后牙支抗有影响,且随严重程度的增加,支抗磨牙位置前移量增加。 Objective To investigate the effects of different degrees of fluorotic teeth on the position of maxillary molars and incisors in adult patients with class Ⅰ extraction during orthodontic treatment process.Methods Sixty orthodontic patients in Department of Orthodontics,Tianjin Municipal Stomatological Hospital From July 2010 to July 2018 were selected and divided into the severe group,moderate group,mild group and control group according to the Dean dental fluorosis classification standard.The Dolphin measurement software was used to screen out the patients meeting the standard.The X-ray skull lateral slices before and after treatment were measured and conducted the comparison analysis.The measurement indexes:(1) the crossing angle between the long axis of the maxillary central incisor and the plane of the palate(PP);(2) the crossing angle between the long axis of maxillary first molar and maxillary PP (U6 to PP);(3) the distance from the incisor edge of the maxillary central incisor to the plane(PTV) perpendicular to the PP;(4) the distance from the apex of the root of the maxillary central incisor to the PTV plane;(5) the distance between the proximal most protruding point of the maxillary first molar and PTV(U6M-Hor);(6) the distance between the lowest point of proximal middle root of maxillary first molar and PTV (U6A-Hor).Results The results of 6 measurement indexes had no statistical differences between the mild group and control group( P >0.05);U6 to PP,U6M-Hor and U6A-Hor had statistical differences between the moderate group and control group( P <0.05);the 6 measurement indexes had statistical differences between the severe group and control group( P <0.05);the 6 measurement indexes had statistical differences between the mild group and moderate group with the severe group,and the 6 measurement indexes had statistical differences between the mild group and moderate group( P <0.05).Conclusion Different degrees of dental fluorosis have the influence on the posterior teeth anchorage,and with the increase of the severity,the forward movement amount of anchorage molar position increases.
