摘要
目的:比较阴式手术与开腹手术对子宫良性肿瘤患者手术应激指标及术后恢复状态的影响。方法:对2015年10月—2018年10月收治的64例子宫良性肿瘤患者的临床资料进行回顾性分析,其中35例行阴式手术(阴式组),29例行传统开腹手术(开腹组)。比较两组患者手术指标与术后恢复情况(手术时间、术中出血量、体温恢复时间、导尿管留置时间、排气时间、住院时间)。对术前24h和术后48h血清应激反应相关指标[白介素-6(IL-6)、C反应蛋白(CRP)、白细胞计数(WBC)、皮质醇(Cor)]水平进行对比。结果:所有患者均顺利完成手术。两组术后体温恢复时间、导尿管留置时间比较,差异无统计学意义(P>0.05);阴式组患者手术时间、术中出血量、排气时间、住院时间均小于开腹组,差异有统计学意义(P<0.05);两组患者术后48hIL-6、CRP、WBC、Cor均较术前24h升高(P均<0.05),且阴式组明显低于对照组(P<0.05)。结论:与腹式手术相比阴式手术治疗子宫良性肿瘤,患者术后恢复更快,手术应激反应与炎性反应更为轻微。
Objective:To compare the effects of vaginal surgery and open surgery on surgical stress indexes and postoperative recovery status in patients with benign uterine tumors.Methods:A retrospective analysis was performed on the clinical data of 64 patients with benign uterine tumors admitted to our hospital from October 2015 to October 2018.Among them,35 patients were given vaginal surgery(vaginal group),and 29 patients were given traditional open surgery(open group).The surgery and postoperative recovery(surgical time,intraoperative blood loss,body temperature recovery time,catheter indwelling time,exhaust time,hospital stay),and levels of serum stress response-related indexes interleukin-6(IL-6),C-reactive protein(CRP),white blood cell count(WBC),cortisol(Cor)at 24 h before surgery and at 48 h after surgery were compared between the two groups.Results:All patients underwent successful operation,and there were no significant differences in the postoperative temperature recovery time and catheter indwelling time between the two groups(P>0.05),and the surgical time,intraoperative blood loss,exhaust time and hospital stay in vaginal group were lower than those in open group(P<0.05).The levels of IL-6,CRP,WBC and Cor in the two groups at 48h after surgery were higher than those at 24 h before operation(P<0.05),but the levels in vaginal group were significantly lower than those in control group(P<0.05).Conclusion:Compared with open surgery,vaginal surgery for benign uterine tumors has faster postoperative recovery and milder surgical stress response and inflammatory response.
