无创呼吸机在急性高原肺水肿的疗效分析 被引量:6

摘要 目的探讨无创呼吸机在急性高原肺水肿中的临床疗效,并评价其应用价值。方法选择该院自2016年1月—2018年2月期间收治的急性高原肺水肿患者157例,按照入院的先后顺序将其分为参照组与观察组,参照组患者66例,观察组患者91例,其中参照组使用持续高流量吸氧方式对患者进行治疗,观察组选择无创呼吸机对患者进行治疗,观察并对比两组患者治疗后的临床效果。结果通过对两组患者施以不同治疗方式可知,观察组患者两肺湿罗音消失时间为(24.76±4.11)h,肺水肿完全消失时间为(23.72±2.99)h,平均住院时间为(3.08±0.63)d;参照组两肺湿罗音消失时间为(76.22±4.78)h,肺水肿完全消失时间为(72.06±2.68)h,平均住院时间为(9.49±1.37)d,两组数据对比差异有统计学意义(t=72.280、104.390、39.300,P<0.05);且观察组患者在无创正压通气治疗后酸碱度(pH)、动脉血氧分压(PaO2)、动脉血氧饱和度(SaO2)以及呼吸频率(RR)的数据,明显优于治疗前的各项数据,差异有统计学意义(t=21.720、21.840、14.300、34.140,P<0.05)。结论在急性高原肺水肿的治疗中,应用无创呼吸机能够快速改善患者的临床症状,更为安全有效,临床疗效显著,值得广泛研究推广。 Objective To investigate the clinical efficacy of noninvasive ventilator in acute high altitude pulmonary edema and evaluate its application value.Methods A total of 157 patients with acute high altitude pulmonary edema admitted to the hospital from January 2016 to February 2018 were enrolled.They were divided into reference group and observation group according to the order of admission.66 patients in the reference group,the observation group patients of 91 cases,the reference group used continuous high-flow oxygen inhalation to treat patients.The observation group selected non-invasive ventilator to treat patients,and observed and compared the clinical effects of the two groups after treatment.Results By treating different groups of patients with different treatment methods,the disappearance time of the two lungs wet rales was(24.76±4.11)h,the time of complete disappearance of pulmonary edema was(23.72±2.99)h,and the average hospital stay was(3.08±0.63)d;the disappearance time of the two lungs wet rales in the reference group was(76.22±4.78)h,the time of complete disappearance of pulmonary edema was(72.06±2.68)h,and the average hospital stay was(9.49±1.37)d.The difference was statistically significant(t=72.280,104.390,39.300,P<0.05);after treatment the patients in the observation group with non-invasive positive pressure ventilation for pH,arterial oxygen partial pressure(PaO2),arterial oxygen saturation(SaO2),and the respiratory rate(RR)data was significantly better than the pre-treatment data,and the difference was statistically significant(t=21.720,21.840,14.300,34.140,P<0.05).Conclusion In the treatment of acute high altitude pulmonary edema,the application of non-invasive ventilator can quickly improve the clinical symptoms of patients,it is safer and more effective,and the clinical curative effect is significant.It is worthy of extensive research and promotion.
