摘要
目的旨在探讨CT、MRI诊断大脑镰及小脑幕硬膜下血肿的应用价值。方法选取我院2016年5月-2018年3月收治的大脑镰或小脑幕硬膜下血肿患者53例,患者均进行了MRI及多层螺旋CT检查,收集53例患者临床资料及影像学资料,与手术结果对照,计算CT、MRI检查对大脑镰及小脑幕硬膜下血肿的诊断符合率。结果MRI对大脑镰及小脑幕硬膜下血肿诊断总符合率为98.11%(52/53),明显低于CT诊断总符合率86.79%(46/53)(P<0.05);CT图像中大脑镰硬膜下血肿,7例呈现细绳索状,6例粗绳索状,血肿近中线侧边缘平直,外侧则表现为弧形,血肿侧对应部位脑回远离中线,脑沟模糊。小脑幕旁硬膜下血肿患者CT图像中3例血肿呈片状,6例呈“Y”形。冠状位MRI扫描对小脑幕硬脑膜下血肿显示较佳,各序列高信号持续时间长,血肿区表现为沿小脑幕分布区呈倒“V”字形高信号,血肿累计大脑镰时,“V”字形高信号则变为“Y”字形。结论CT、MRI均可直观显示大脑镰及小脑幕硬膜下血肿的典型图像特征,MRI诊断大脑镰及小脑幕硬膜下血肿符合率较CT高,临床可根据需求选择检查方式。
Objective To investigate the diagnostic value of CT and MRI in cerebral falx and tentorial subdural hematoma.Methods 53 patients with cerebral falciform or tentorial subdural hematoma admitted to our hospital from May 2016 to March 2018 were selected.The patients were examined by MRI and multi-slice spiral CT.The clinical data and imaging data of 53 patients were collected and compared with the results of operation.The diagnostic coincidence rate of CT and MRI for cerebral falciform and tentorial subdural hematoma was calculated.Results The total coincidence rate of MRI in diagnosis of falx cerebri and tentorial subdural hematoma was 98.11%(52/53),which was significantly lower than that of CT(86.79%(46/53)(P<0.05).Falx subdural hematoma in CT images,7 cases showed a string shaped,6 cases of thick funiform hematoma,midline side straight edges,lateral performance of arc,the hematoma side corresponding position away from the midline sulcus gyrus,fuzzy.In CT images of patients with paratentorial subdural hematoma,3 cases showed patchy hematoma,6 cases showed"Y"shape.Coronal MR imaging showed better subdural hematoma in tentorium cerebellum.The high signal duration of each sequence was long.The hematoma area showed inverted"V"shape high signal along tentorium cerebellum.When the hematoma accumulated in falx cerebri,the"V"shape high signal changed to"Y"shape.Conclusion CT and MRI can display the typical image features of cerebral falx and tentorium of cerebellum subdural hematoma,MRI diagnosis of cerebral falx and tentorium of cerebellum subdural hematoma coincidence rate higher than CT,can be selected according to the needs of clinical examination.
