不同碘盐暴露儿童碘营养及甲状腺状况分析 被引量:10

Analysis of iodine nutrition and thyroid status in children exposed to different iodized salts

摘要 目的探索食用不同碘盐儿童碘营养、甲状腺情况及相关影响因素。方法在天津市碘盐覆盖率低的地区随机抽取2个学校,班级整群抽样法调查选取453名儿童,采用现场半定量试剂盒测定盐碘,采用B超法检查甲状腺情况,采用化学发光法检测甲状腺激素水平,采用砷铈催化分光光度法检测尿碘。结果在此项研究中,碘盐暴露儿童尿碘中位数为164.90μg/L,未加碘食盐暴露儿童尿碘中位数为168.30μg/L,碘盐暴露儿童甲状腺肿大率为1.78%,未加碘食盐暴露儿童甲状腺肿大率为2.20%。两组儿童的碘营养情况、甲状腺肿大情况以及大部分甲状腺激素或抗体水平差异无统计学意义(P均>0.05),未加碘食盐暴露儿童血清TPOAb水平高于碘盐暴露儿童(P<0.05),通过多元线性回归分析发现碘盐种类、性别、年龄为TPOAb的影响因素(P均<0.05)。结论天津市不同碘盐暴露儿童尿碘中位数处于适宜水平,食用未加碘食盐可能会干扰儿童甲状腺抗体水平,未发现其与甲状腺肿大情况之间的关系,还需要进一步研究。 Objective To explore the iodine nutrition,thyroid function and influencing factors of children exposed to different iodized salts.Methods In Tianjin,two schools were randomly selected in areas with low iodized salt coverage.A total of 453 children were selected by class cluster sampling method.Iodine concentration in salt was determined on-site by a semi-quantitative detection kit.The thyroid function was examined by B ultrasonic method.Detection of thyroid hormone levels was carried out by chemiluminescence method.Urinary iodine concentration was determined by“As3+-Ce4+catalytic spectrophotometry”.Results In this study,median urinary iodine of children exposed to iodized salt was 164.90μg/L,while median urinary iodine of children exposed to non-iodine salt was 168.30μg/L.The goiter rate of children exposed to iodized salt was 1.78%.The goiter rate of children exposed to non-iodine salt was 2.20%.There was no significant difference in iodine nutrition,thyroid enlargement,and most thyroid hormones or antibody levels between the two groups(all P>0.05).Children with non-iodine salt exposure had higher levels of TPOAb than children exposed to iodized salt(P<0.05).Multivariate linear regression analysis found that the type of iodized salts,sex and age were the influencing factors of TPOAb levels(all P<0.05).Conclusion In Tianjin,the median urinary iodine level of children exposed to different iodized salts was at an appropriate level.Using non-iodine salt may interfere with the secretion of thyroid hormone in children.A positive correlation between the intake of non-iodine salt and thyroid enlargement was not found in this study,which needs to be further investigated.
