摘要
目的 探讨非扩髓交锁髓内钉内固定治疗胫骨骨干骨折延迟愈合的危险因素。方法 回顾性收集沈阳中大骨科医院2012年1月-2018年12月收治的胫骨骨干骨折患者265例,根据术后是否存在骨折延迟愈合分为延迟组82例和对照组183例。比较两组患者的基线资料及治疗情况,并探讨折延迟愈合的危险因素。结果 单因素Logistic回归分析显示,年龄、体重指数(BMI)、过早活动、内固定质量、骨折端不稳、功能锻炼与骨折延迟愈合有关(P<0.05)。多因素Logistic回归分析显示,在剔除年龄和BMI后,过早活动、内固定质量、骨折端不稳、功能锻炼仍与骨折延迟愈合有关(P<0.05)。结论 对于非扩髓交锁髓内钉内固定治疗胫骨骨干骨折的患者,如果存在术后过早活动、内固定不佳、骨折端不稳和未进行功能锻炼等情况,应警惕延迟愈合的发生。
Objective To investigate the risk factors of delayed union of tibial shaft fractures treated with unreamed interlocking intramedullary nails internal fixation. Methods A total of 265 cases of tibial shaft fractures admitted to our hospital from January 2012 to December 2018 were retrospectively collected. According to the existence of delayed union after operation, all patients werewere divided into delayed group (N=82 cases) and control group (N=183 cases). The baseline data and treatment of the two groups were compared, and the risk factors of delayed healing were discussed. Results Univariate logistic regression analysis showed that age, BMI, early activity, internal fixation quality, instability of fracture end and functional exercise were related to delayed union of fracture (P<0.05). Multivariate logistic regression analysis showed thatearly activity, internal fixation quality, instability of fracture end and functional exercise were still related to delayed union of fracture after age and BMI were excluded (P<0.05). Conclusion For patients with tibial shaft fracture treated with unreamed interlocking intramedullary nail internal fixation, if there are early activities, poor internal fixation, unstable fracture end and no functional exercise after operation, the occurrence of delayed union should be vigilant.
