摘要
目的针对下肢静脉曲张术后复发的原因进行预防及治疗。方法选取济南市市中区人民医院外科2017年9月至2018年12月收治的36例下肢静脉曲张术后复发患者资料,包括首次手术方式、复发原因、治疗方案及效果。结果术后复发最常见首次手术是大隐静脉腔内全程激光闭合术(50.00%),其次为聚桂醇泡沫硬化剂注射治疗(27.78%)以及大隐静脉高位结扎+剥脱术(22.22%)。最常见复发原因是大隐静脉主干或属支残留,其次是交通静脉瓣膜功能不全。结论下肢静脉术后复发最常见原因是首次手术方式不彻底,静脉主干或分支残留以及交通静脉未处理完全。手术彻底治疗反流以及完全处理交通静脉是预防以及治疗下肢静脉术后复发的关键措施。
Objective Prevention and treatment of recurrence of varicose veins of lower extremities after operation.Methods The data of 36 patients with recurrence of varicose veins of lower extremities admitted to our hospital from September 2017 to December 2018 were selected,including the first operation method,recurrence causes,treatment plan and effect.Results The most common postoperative recurrence was the endovenous laser closure of the great saphenous vein (50.00%),followed by polyglycerol foam sclerosing agent injection therapy (27.78%) and high saphenous vein ligation + stripping (22.00%).The most common cause of recurrence was residual trunk or branch of great saphenous vein,followed by insufficiency of communicating vein valve.Conclusion The most common causes of recurrence after lower extremity venous surgery are incomplete first operation,residual main or branch veins,and incomplete treatment of communicating veins.Surgical treatment of reflux and thorough management of communicating veins are the key measures to prevent and treat recurrence of lower extremity veins after operation.
