摘要
目的:调查护士经历不良事件后作为第二受害者的痛苦和支持现状,分析其影响因素并提出临床实践建议。方法:于2018年5-6月采用方便抽样法选取四川省2所三级甲等医院经历不良事件的258名护士为研究对象,采用自行设计的一般资料问卷、不良事件特征问卷,以及第二受害者经验及支持量表进行调查。结果:护士第二受害者痛苦得分为(3.22±0.93)分,组织支持为(2.59±0.56)分,离职倾向为(3.38±1.26)分,缺勤为(2.85±1.14)分,非工作相关支持为(2.18±1.03)分。差异显著因素对第二受害者组织支持、离职倾向、缺勤、非工作相关支持变异的解释程度分别为4.7%、1.6%、6.3%、2.5%。结论:护士经历不良事件后的第二受害者痛苦水平较高,得到的支持水平较低,并受到多因素影响。医院管理者应当客观认识护士的第二受害者痛苦,并提供正式组织支持和专业心理辅导,促进护士职业健康和生活幸福,从而改善医疗护理质量。
Objectives: To investigate the experiences of the nurses as the second victims after the adverse events. Methods: The convenient sampling method was applied. Totally 258 nurses from two general hospitals in Sichuan were included. The Second Victim Experience and Support Tool (SVEST) and Adverse Event Characteristics Questionnaire (AECQ) were performed in this study. Results: The score of the distress was (3.22±0.93), the organizational support was (2.59±0.56), turnover intentions was (3.38±1.26), absenteeism was (2.85±1.14), non-work-related support was (2.18±1.03) respectively. The variables that could explain the variation of dependent variables were the absenteeism (6.3%), organizational support (4.7%), non-work-related support (2.5%) and turnover intentions (1.6%). Conclusion: Nurses experience second victim distress after the adverse events, but they only accept low level support from the working environment. Hospital managers should provide high level of organizational support and professional psychological counseling to nurses in order to promote the nurses health.
