摘要
目的:探讨益气升阳法对气虚下陷型胸痹心痛患者的临床疗效。方法:选取2017年9月-2018年9月心病科门诊气虚下陷型胸痹心痛患者共86例。按随机数字表法分为治疗组(43例)和对照组(43例),对照组采用阿司匹林肠溶片、他汀、硝酸异山梨酯等常规西药治疗,治疗组在常规治疗的基础上根据舌苔、脉象等给予益气升阳举陷的方药,如黄芪、党参、升麻、柴胡、桔梗、桂枝、贯叶金丝桃、刺五加等治疗,两组疗程为14 d。比较两组患者治疗前后的胸痹心痛证候积分、血脂及左心室射血分数(Left Ventricular Ejection Fraction,LVEF)、左心室舒张末期内径(Left Ventricular End Diastolic Diameter,LVEDD)的变化,并比较两组治疗的有效率,不良反应的发生率。结果:治疗组的胸痹心痛证候较对照组有明显改善(P<0.05),治疗组胸痹心痛证候评分明显降低均低于对照组(P<0.05),治疗组有效率高于对照组。统计学差异两组不良反应的发生率分别为4.7%(2/43)、2.3%(1/43),差异无统计学意义(P>0.05);治疗组总胆固醇(Total Cholesterol,TC)、甘油三酯(Triglyceride,TG)、低密度脂蛋白胆固醇(Low Density Lipoprotein Cholesterol,LDL-C)低于对照组,高密度脂蛋白胆固醇(High Density Lipoprotein Cholesterol,HDL-C)含量明显高于对照组,差异有统计学意义(P<0.05);两组患者治疗后LVEF较治疗前升高,且治疗组高于对照组(P<0.05),LVEDD较治疗前降低,且治疗组低于对照组(P<0.05)。结论:益气升阳法可有效缓解气虚下陷型胸痹心痛患者的胸闷、心悸、气短、失眠症状,改善患者血脂及心功能水平,提高治疗效果。
Objective:To explore the clinical efficiency of the Yiqi Shengyang therapy(益气升阳法)on chest pain and heart pain of the Qixu Xiaxian type(气虚下陷型).Methods:86 patients were divided into the treatment group(43 cases)and the control group(43 cases).The control group was given aspirin enteric-coated metformin hydrochloride,statins,different yamanashi nitrate ester.The treatment group was given the Yiqi Shengyang Juxian prescription more.Results:Syndromes of chest arthralgia and heartache were significantly improved in the treatment group(P<0.05),and efficiency was higher than the control group.The adverse reactions in the two groups were 4.7%(2/43)and 2.3%(1/43),respectively,with no statistically significant difference(P>0.05).The total cholesterol(TC),triglyceride(TG)and lowdensity lipoprotein cholesterol(LDL-L)in the treatment group were lower than those in the control group,and the high-density lipoprotein cholesterol(HDL-L)content was significantly higher than that in the control group,with statistically significant differences(P<0.05).After treatment,LVEF in the two groups was more increased than before treatment,and LVEF in the treatment group was more increased(P<0.05).LVEDD was more decreased that before treatment(P<0.05),and that in the treatment was more decreased.Conclusion:The Yiqi Shengyang therapy was effective,can effectively relieve the symptoms,improve the level of blood lipid and cardiac function,and improve the therapeutic effect.
