摘要
目的观察OSAHS合并不同程度亚临床甲状腺功能减退患者应用持续正压通气(CPAP)和/或左甲状腺素钠片治疗后能否改善预后。方法 OSAHS患者进行甲功检查,按TSH是否大于10mIU/L分为2组,TSH>10mIU/L组给予CPAP+左甲状腺素钠片治疗;TSH4~10mIU/L患者组根据是否有临床症状再分为2组,有症状组予CPAP+左甲状腺素钠片治疗组,无症状组予CPAP治疗组。结果(1)TSH4~10mIU/L有症状组,治疗后AHI、ODI4、最长暂停时间明显低于治疗前,最低血氧饱和度、平均血氧饱和度明显高于治疗前,差异具有显著统计学意义(P<0.01);(2)TSH>10mIU/L时,治疗后AHI、ODI4、最长暂停时间明显低于治疗前,最低血氧饱和度、平均血氧饱和度明显高于治疗前,差异具有显著统计学意义(P<0.01)。结论 OSAHS合并亚临床甲状腺功能减退症患者,积极CPAP+左甲状腺素钠片治疗可明显改善OSAHS的预后。
Objective To observe whether the prognosis of obstructive sleep apnea hypopnea syndrome(OSAHS) patients with subclinical hypothyroidism can be improved by taking continuous positive airway pressure (CPAP) and / or levothyroxine sodium tablets for OSAHS combined with different degrees of subclinical hypothyroidism. Methods Thyroid function tests were performed in patients with OSAHS. Then,patients with OSAHS combined with hypothyroidism were divided into two groups according to whether TSH was above 10 mIU/L. Patients with TSH >10 mIU/L was treated with CPAP and levothyroxine sodium tablets. Patients with TSH 4-10 mIU/L group were further divided into two groups based on whether they had clinical symptoms. The research group received CPAP and levothyroxine sodium tablet treatment, and the control group took CPAP treatment. Results(1)AHI, ODI4, and the longest pause time were significantly lowered in the TSH 4-10mIU/L symptomatic group. The lowest oxygen saturation and mean oxygen saturation were significantly higher than before, the differences were statistically significant(P < 0.01).(2)The maximum pause time of AHI and ODI4 was significantly loweed in the TSH >10 mIU/L group. The lowest oxygen saturation and mean oxygen saturation were significantly higher than before,and the differences were statistically significant(P < 0.01). Conclusion The prognosis of OSAHS can be significantly improved by taking CPAP and levothyroxine sodium tablets actively for OSAHS combined with subclinical hypothyroidism patients.
