摘要
目的探讨降低智能化药房调配差错率的干预措施和优化方案。方法采用自身对照,分析我院门诊西药房优化前(2015年1月)和优化后(2015年5月)1周工作日共计27867张处方出现的98例调剂差错的原因和类型,并评价优化措施的效果。结果自动发药机引起的调剂数量差错为主要因素,占调剂数量差错的78.5%。针对自动发药机的出现的调剂差错从轨道卡药、机器缺药、瓶装药品轨道设计问题、传动带卡药、药盒粘连、机器滞后等6个方面进行原因分析。对以上原因采用优化特殊药品轨道,加药臂加装泡沫条等一系列措施来提高自动发药机的准确性。实施优化方案后,自动发药机的调剂差错率明显降低,由改善前的4.61‰下降至改善后的2.64‰,改善前后差异有统计学意义(P<0.01)。结论降低智能发药机调剂差错的实践研究有效解决了新的工作模式存在的问题,达到保证患者的用药安全的目的。
Objective To discussion on intervening measures and optimized proposals of reducing the dispensing error rate of intelligent pharmacy. Methods 27,867 prescriptions from outpatient pharmacy were collected and were checked with the method of self-contract method. All these prescriptions are made within one week of workday during January 2015 before taking optimized measures and that in May 2015 after taking optimized measures. Result The major error is caused by intelligent dispensing machine,which contruibuted to 78.5% of all the errors.We analyzed causes that led to errors of intelligent dispensing machine, including medicines getting stuck on the track, lackage of medicines, track design of bottle medicines,medicines getting stuck on the driving belt, medicine boxes pasting on each other and lagging dispensing machine. The accuracy of automatic dispensing machine was improved by optimizing track of special medicines and increasing machine arms and foam articles, etc. and we evaluated the effect of optimized measures at the same time. The error rates of intelligent dispensing machine dramatically reduced from 4.61‰ to 2.64‰ after taking optimized measures. and this result showed a statistically difference(P<0.1). Conclusion Intervening measure and optimized proposal of reducing the error rate of intelligent dispensing machine can effectively solve the problems of our recent working system and ensure the pharmacy safety of patients.
