高血压脑出血患者TIMP-1、MMP-9、NSE、IL-18水平变化对神经功能影响 被引量:33

Effects of Changes in TIMP-1,MMP-9,NSE and IL-18 Levels on Neurological Function in Patients with Hypertensive Intracerebral Hemorrhage

摘要 目的探讨高血压脑出血患者血清基质金属蛋白酶9(MMP-9)、基质金属蛋白酶组织抑制剂-1(TIMP-1)、血清神经元特异性烯醇化酶(NSE)、白细胞介素-18(IL-18)的变化与病情及神经功能缺损的关系。方法选取我院收治的高血压脑出血患者100例(高血压脑出血组)、同期健康体检对象100例(对照组),检测两组的TIMP-1、MMP-9、NSE、IL-18水平并进行比较;按照美国国立卫生研究院卒中量表评分(NIHSS)对高血压脑出血组进行分组,治疗2周前后对上述指标进行对比。结果高血压脑出血组患者的TIMP-1、MMP-9、NSE、IL-18测定值均显著高于对照组,差异均具有统计学意义(F=7.62~23.65,均P<0.05);重度、中度的高血压脑出血患者的TIMP-1、MMP-9、NSE、IL-18测定值均显著高于轻度组(F=15.45~23.60,P<0.05),重度的高血压脑出血患者的TIMP-1、MMP-9、NSE、IL-18测定值均显著高于中度组(均P<0.05);治疗2周后的高血压脑出血患者的TIMP-1、MMP-9、NSE、IL-18测定值均显著的低于治疗前水平(均P<0.05);高血压脑出血患者治疗前的TIMP-1、MMP-9、NSE、IL-18测定值与患者NIHSS评分均呈正相关(P<0.05)。结论高血压脑出血患者TIMP-1、MMP-9、NSE、IL-18水平升高显著,并且可以反应患者的病情变化、神经功能缺损程度。 Objective To investigate the changes in plasma matrix metalloproteinase-9(MMP-9),tissue inhibitor of metalloproteinase-1(TIMP-1),serum neuron specific enolase(NSE),and interleukin-18,and their relationship with neurological deficits in patients with hypertensive intracerebral hemorrhage.Methods 100 cases of hypertensive intracerebral hemorrhage(hypertensive intracerebral hemorrhage group)and 100 healthy subjects(control group)were enrolled in our hospital.TIMP-1,MMP-9,NSE and IL-18 were detected and compared in the two groups.The hypertensive intracerebral hemorrhage group was subgrouped into mild,moderate and severe group according to the National Institutes of Health Stroke Scale(NIHSS),and changes of above indicators were observed 2 weeks after treatment.Results The estimated values of TIMP-1,MMP-9,NSE and IL-18 in the hypertensive intracerebral hemorrhage group were significantly higher than those in the control group,the difference was statistically significant(P<0.05);The estimated values of MMP-9,NSE and IL-18 were significantly higher in patients with severe and moderate than those in the mild group(P<0.05).The estimated values of TIMP-1,MMP-9,NSE and IL-18 in patients with severe hypertensive intracerebral hemorrhage were significantly higher than those in the moderate group(P<0.05).The estimated values of TIMP-1,MMP-9,NSE and IL-18 in hypertensive intracerebral hemorrhage patients after 2 weeks of treatment were significantly lower than those before treatment(P<0.05).In hypertensive intracerebral hemorrhage patients,the values of TIMP-1,MMP-9,NSE and IL-18 before treatment were significantly positively correlated with NIHSS scores(P<0.05).Conclusions The levels of TIMP-1,MMP-9,NSE and IL-18 in hypertensive intracerebral hemorrhage patients are significantly increased,and can reflect the patient’s condition and degree of neurological deficit.
