动脉瘤性蛛网膜下腔出血后垂体前叶激素水平变化的临床研究 被引量:6

摘要 目的探讨动脉瘤性蛛网膜下腔出血(aneurysmal subarachnoid hemorrhage,aSAH)患者垂体前叶功能减退发生情况,与临床病情的相关性及随访期间变化情况。方法回顾性分析48例aSAH患者发病后3天、1周、3周、3月和6月的垂体前叶激素分泌情况,并分析与改良Fisher分级、Hunt-Hess分级的相关性。结果aSAH后3月和6月出现ACTH和GH降低,改良Fisher分级4级患者ACTH水平降低,改良Fisher分级3级和4级患者GH水平降低,Hunt-HessⅣ~Ⅴ级患者GH水平降低。结论aSAH恢复期出现ACTH和GH降低,与改良Fisher分级、Hunt-Hess分级有相关性。 Objective To explore the risk of anterior pituitary hormones after aneurysmal subarachnoid hemorrhage(aSAH)and to evaluate its correlation with clinical data and follow-up.Methods 48 cases of patients with aSAH were retrospectively analysed.Serum adrenocorticotrophic hormone(ACTH),growth hormone(GH),thyroid stimulating hormone(TSH),follicle-stimulating hormone(FSH),luteinizing hormone(LH),and prolactin(PRL)were dynamic tested 3days,1week,3weeks,3months,6months after the onset.Results ACTH and GH deficiency were the most frequent disorder at the 3months and 6months follow-up.ACTH and GH deficiency were related to Fisher grade and Hunt-Hess system.Conclusion The incidence of anterior pituitary hormones after aSAH was rear and related to the severity of patientsand affect the prognosis.
