摘要
新中国成立以来,中国的社会医疗保险制度从计划经济时期的公费医疗和劳保医疗制度演变而来,在其演变的过程中制度的身份差异和地域差异不断加剧,当前最明显的特点是制度的“碎片化”及伴随制度的不公平和不平等问题。完善全民医保体系是建设中国特色医疗保障制度的题中之义,而当前学术界对全民医保的实现路径尚未达成共识。文章认为首先要保障公民平等地享有健康保障权,这是实现全民医保的逻辑起点。其次,在统一医保制度的过程中,保证参保者都能平等地参与到制度中来,缴费端应该按照公平原则,各尽所能;在待遇享受上,参保者能够平等地享有医保报销待遇。与此同时,要注重医疗服务资源的均衡配置,逐步缩小地区间的差距。
China’s social health insurance system has evolved from the public health and labor insurance systems in the planned economy.In the process of its evolution,the identity differences and regional differences of the system have been intensifying.At present,the most obvious feature is the“fragmentation”of the system and the accompanying institutional inequities and inequalities.Improving the national health insurance system is the key to the construction of the health security system with Chinese characteristics,but the academic circles have not reached a consensus on the path to the realization of the national health insurance.This paper argues that the first step is to guarantee citizens’equal right to health insurance,which is the logical starting point of the realization of universal health insurance.Secondly,in the process of unifying the health insurance system,the participants are guaranteed to participate in the system equally.The contributors should do their best according to the principle of fairness.In terms of treatment enjoyment,the participants can enjoy the treatment of healthinsurance reimbursement equally.At the same time,we should pay attention to the balanced allocation of health service resources and gradually narrow the gap between regions.
