摘要
目的评价三氧膏的体外抗真菌作用及其对皮肤真菌感染的临床治疗效果,为进一步推广应用提供科学依据。方法(1)以白色念珠菌、须癣毛癣菌、红色毛癣菌和石膏样小孢子菌等4种临床常见致病真菌为研究对象,采用纸片法及微量稀释法,测定三氧膏、酮康唑乳膏和紫草膏的抑菌圈直径及最低抑菌浓度。(2)将2017年5月—2018年5月我院收治的326例真菌感染(足癣)患者随机分为2组:三氧膏治疗组、酮康唑乳膏治疗组,比较两组的临床疗效。结果(1)抑菌圈直径:三氧膏与酮康唑乳膏对4种致病真菌的直径范围为26.62~31.64 mm、29.44~34.49 mm,三氧膏略低于酮康唑乳膏,均表现为高度敏感;紫草膏≤9.20 mm,表现为无抗菌作用。(2)最低抑菌浓度:对4种致病真菌的MIC值,三氧膏为0.47~15.0μg/mL,酮康唑乳膏为0.12~15.0μg/mL,紫草膏均>120.0μg/mL。抗菌作用强弱为酮康唑乳膏>三氧膏>紫草膏。(3)治疗足癣患者的临床疗效:三氧膏治疗组的总有效率为85.28%,与酮康唑乳膏治疗组的疗效相当。结论三氧膏具有明显的体外抗真菌作用与临床疗效,值得进一步深入研究和推广应用。
Objective To provide scientific basis for the further popularization and application of triple oxygen ointment by evaluating its antifungal effect and clinical efficacy on skin fungal infection.Methods(1)The inhibition zone diameter and minimum inhibitory concentration of triple oxygen ointment,ketoconazole and Shikong plaster on Candida albicans,Trichophyton mentagrophytes,Trichophyton rubrum and Microsporum gypsum-like bacteria were measured by slip method and micro-dilution.(2)326 patients with fungal infection(tinea pedis)admitted to our hospital from May 2017 to May 2018 were randomly divided into two groups:triple oxygen ointment treatment group and ketoconazole treatment group.The clinical efficacy of two drugs was compared.Results(1)The inhibition zone diameters of triple oxygen ointment and ketoconazole on four kinds of fungi were 26.62-31.64 mm and 29.44-34.49 mm,respectively.Four kinds of fungi were all sensitive to both antifungal drugs.The inhibition zone diameters of Shikong plaster on four kinds of fungi were less than 9.20 mm,which did not show antifungal effect.(2)The MICs of triple oxygen ointment,ketoconazole and Shikong plaster on four kinds of fungi were 0.47-15.0μg/mL,0.12-15.0μg/mL and>120.0μg/mL,respectively.The antimicrobial activity was ketoconazole>triple oxygen ointment>Shikong plaster.(3)The clinical efficacy of triple oxygen ointment on tinea pedis was 85.28%,which is comparable to ketoconazole ointment.Conclusion Triple oxygen ointment has obvious antifungal effect in vitro and clinical efficacy on skin fungal infection,which is worth further study and application.
