摘要
目的对BPPV诊疗系统复位与手法复位治疗良性阵发性位置性眩晕(BPPV)的临床疗效研究进行Meta分析,为选择耳石复位的方法提供循证医学依据。方法检索BPPV诊疗系统复位(试验组)或手法复位(对照组)治疗BPPV的随机对照试验,筛选Jadad评分≥3分的文献,采用Stata 12.0软件进行Meta分析。结果共纳入7个随机对照试验807例患者,试验组复位1次、复位次日、复位后1周(包括1周治愈率和1周有效率)较对照组均有明显获益,差异具有统计学意义(RD=0.16,95%CI=0.06~0.26,P<0.05;RD=0.32,95%CI=0.20~0.43,P<0.05;RD=0.24,95%CI=0.09~0.40,P<0.05;RD=0.11,95%CI=0.06~0.15,P<0.05)。结论试验组的短期疗效优于对照组,建议临床上对BPPV患者采用BPPV诊疗系统进行复位。
[Objective]To evaluate the clinical effect of reduction with the diagnosis and treatment system for benign pa-roxysmal positional vertigo(BPPV)versus manual reduction in the treatment of BPPV through a Meta-analysis,and to provide an evidence-based medicine basis for canalith repositioning maneuvers.[Methods]Related databases were searched for randomized controlled trials(RCTs)on reduction with the BPPV diagnosis and treatment system(experimental group)or manual reduction(control group)in the treatment of BPPV,and the articles with a Jadad score of≥3 were screened out.Stata 12.0 was used to perform the meta-analysis.[Results]A total of 7 RCTs with 807 patients were included.Compared with the control group,the experimental group had significantly greater benefits after one reduction,on the next day after reduction,and at 1 week after reduction in terms of 1-week cure rate and 1-week response rate(risk difference(RD)=0.16,95%confidence interval(CI)=0.06-0.26,P<0.05;RD=0.32,95%CI=0.20-0.43,P<0.05;RD=0.24,95%CI=0.09-0.40,P<0.05;RD=0.11,95%CI=0.06-0.15,P<0.05).[Conclusion]The experimental group has a better short-term clinical outcome than the control group,and therefore,reduction with the BPPV diagnosis and treatment system is recommended for patients with BPPV in clinical practice.
