经皮微创钢板内固定术对胫骨中下段骨折患者术中出血量及骨折愈合时间的影响 被引量:5

Effect of minimally invasive percutaneous plate osteosynthesis on intraoperative bleeding volume and fracture healing time in patients with middle and lower tibial fracture

摘要 目的研究经皮微创钢板内固定术对胫骨中下段骨折患者术中出血量及骨折愈合时间的影响。方法将2017年1月—2019年1月我院收治的140例胫骨中下段骨折患者,根据随机数字表法分为观察组及对照组各70例。对照组采用交锁髓内钉治疗,观察组使用经皮微创钢板内固定术治疗,对比分析2组患者的血流动力学变化、骨折愈合时间及手术指标。结果观察组术中出血量低于对照组,手术及愈合时间均短于对照组,差异具有统计学意义(P<0.05)。手术进行30 min时观察组MAP水平低于对照组,差异具有统计学意义(P<0.05);SpO2水平略高于对照组,但差异无统计学意义(P>0.05)。结论使用经皮微创钢板内固定术治疗胫骨中下段骨折,具有骨折愈合快、术中出血量小、对骨折端血供影响小等优点,利于预后恢复,值得临床推广。 Objective To study the effect of minimally invasive percutaneous plate osteosynthesis(MIPPO)on intraoperative bleeding volume and fracture healing time in patients with middle and lower tibial fracture.Methods 140 patients with middle and lower tibial fracture in the hospital from January 2017 to January 2019 were divided into observation group and control group by random number table,with 70 cases in each group.Control group was given interlocking intramedullary nail,and observation group underwent MIPPO.The changes of hemodynamics,fracture healing time and surgical indexes were compared between two groups.Results The intraoperative bleeding volume of observation group was lower than that of control group,the operation and healing time in observation group was shorter than that in control group(P<0.05).After surgery for 30min,the level of MAP in observation group was lower than that in control group(P<0.05),and the level of SpO2 in observation group was slightly higher than that in control group,but the difference was not statistically significant(P>0.05).Conclusion MIPPO in treatment of patients with middle and lower tibial fracture has advantages of quick fracture healing,less intraoperative bleeding volume and small effect on blood supply of fracture ends,it can promote prognostic recovery,which is worthy of clinical promotion.
