超声在活动期强直性脊柱炎患者骶髂关节评估中初步应用 被引量:6

Preliminary Application of Ultrasound in Sacroiliac Joint Assessment for Patients with Active Ankylosing Spondylitis

摘要 目的:探讨超声在评价活动期强直性脊柱炎患者骶髂关节血流中的临床应用价值。方法:超声检查32例活动期强直性脊柱炎患者的58个骶髂关节和48例静止期强直性脊柱炎患者86个骶髂关节。能量多普勒超声显示骶髂关节血流,彩色多普勒超声测量两组血流阻力指数(RI)。比较两组血流显示率、血流分级、血流分布、血流性质、舒张期血流反向、RI指标。结果:活动组骶髂关节血流显示率(51/58,87.9%)与静止期组(55/86,64%)比较有统计学意义(P=0.001)。活动组以2级血流信号为主(39/58,67.2%),而静止组的以0及1级血流为主(84/86,97.7%),两组间血流分级差异有统计学意义(P=0.000)。在58个活动期骶髂关节中,38个出现了关节内血流(65.5%),在86个静止期骶髂关节中,16个出现了关节内血流(18.6%)。两组间在血流分布上差异具有统计学意义(P=0.000)。单独舒张期反向血流信号这一指标,组间不存在统计学上差异(P=0.134)。舒张期反向血流信号指标联合血流性质指标,组间的差异具有统计学上意义(P=0.001)。活动组RI值低于非活动组(P=0.000)差异具有统计学意义。结论:超声可显示骶髂关节内异常血流信号,为强直性脊柱炎患者活动性评估提供依据。 Objective:To explore the clinical application value of ultrasound in the evaluation of sacroiliac joint blood flow in patients with active ankylosing spondylitis.Methods:Ultrasound examination was performed on 58 sacroiliac joints in 32 patients with active ankylosing spondylitis and 86 sacroiliac joints in 48 patients with inactive ankylosing spondylitis.Power doppler ultrasound showed sacroiliac joint blood flow and color doppler ultrasound was used to measure the vascular resistance index(RI)of the two groups.The blood flow display rate,blood flow grading,blood flow distribution,blood flow properties,diastolic flow reversal,and RI index were compared between the two groups.Results:The display rate of sacroiliac joint blood flow(51/58,87.9%)in the active group was compared with that in the inactive group(55/86,64%),the difference of which was statistically significant(P=0.001).The active group was mainly grade 2 blood flow signals(39/58,67.2%),while the inactive group was mainly grade 0 and 1 blood flow(84/86,97.7%),suggesting statistical significance concerning the difference in blood flow grading between the two groups(P=0.000).Among the 58 active sacroiliac joints,38 had intra-articular blood flow(65.5%);and among the 86 inactive sacroiliac joints,16 had intra-articular blood flow(18.6%),suggesting statistical significance concerning the difference in blood flow distribution between the two groups(P=0.000).There was no statistical significance concerning the difference in retrograde diastolic flow signals between the two groups(P=0.134).There was statistical significance concerning the differences in retrograde diastolic flow signals and blood flow properties between the two groups(χ^2=10.983,P=0.001).The RI value of the active group(0.66±0.09)was lower than that of the inactive group(0.83±0.09;t=-6.234,P=0.000),the difference of which was statistical significance.Conclusion:Ultrasound can show abnormal blood flow signals in the sacroiliac joint,which provides a basis for activity evaluation of patients with ankylosing spondylitis.
