摘要
目的:探究早期吞咽功能训练对改善吞咽困难恢复效果的影响。方法:选取我院2017年5月至2018年12月收治的67例脑卒中患者作为研究对象,均经临床确诊且符合洼田饮水试验≥Ⅲ级者,按照随机数字表法分为对照组(n=34,采用常规康复训练法)和研究组(n=33,在对照组基础上采用早期吞咽功能训练)。观察比较两组的治疗效果。治疗前后,采用临床中经典的洼田饮水试验及口中进食功能量表(FOIS)评价吞咽障碍的恢复情况,并采用简化巴氏指数(BI)评价患者生活活动能力(ADL)的改善情况。结果:研究组治疗有效率为96.97%(32/33),高于对照组的79.41%(27/34),差异有统计学意义(P<0.05);与治疗前比,两组治疗后FOIS及BI指数得分均升高,洼田饮水实验评分降低(P<0.05);治疗后与对照组比,研究组FOIS及BI指数得分均升高,洼田饮水实验评分降低(P<0.05)。结论:早期吞咽功能训练能够有效改善患者的吞咽功能,提高患者日常生活活动能力,疗效理想。
Objective:To observe the effect of early swallowing function training on dysphagia recovery.Methods:A total of 67 stroke patients from May 2017 to December 2018 were selected as the subjects of the study.According to the random number table method,they were divided into the control group(n=34,routine rehabilitation training method)and the study group(n=33,using early swallowing function training on the basis of control group).The therapeutic effects of the two groups were observed and compared,and the recovery of swallowing disorder was evaluated by water swallowing test and Functional Oral Intalce Scale(FOIS)before and after treatment.Simplified Barthel index(BI)was used to evaluate the improvement of activity of daily living(ADL)in patients.Results:The effective rate of treatment was 96.97%(32/33)in the study group,higher than 79.41%(27/34)in the control group(P<0.05).Compared with before-treatment,the scores of FOIS and BI in the two groups increased after treatment,while the score of water swalling test decreased(P<0.05).After treatment,the scores of FOIS and BI in the study group were higher than those in the control group(P<0.05),while the score of water swallowing test was lower in the study group(P<0.05).Conclusion:Early swallowing function training can effectively improve patients′swallowing function,improve patients′activity of daily living,and the curative effect is ideal.
