滨海新区糖尿病并发症的发病情况及其影响因素分析 被引量:2

摘要 目的探讨滨海新区糖尿病并发症发病情况及影响因素,为各级预防提供依据。方法选取2015年1月1日~2017年12月30日在海滨新区七家医院就诊的糖尿病患者24697例,根据是否存在并发症分为伴有并发症组(6216例)和不伴有并发症组(18481例),比较两组患者的性别、年龄、民族、职业、家庭住址等一般资料和临床资料,分析可能影响糖尿病并发症的危险因素。结果①两组患者性别、年龄、职业以及糖尿病诊断分型情况比较,差异有统计学意义(P<0.05);②多因素Logistics回归分析显示,年龄(OR=1.011,95%CI=1.000~1.020)、2型糖尿病(OR=2.650,95%CI=1.137~6.160)以及职业为国家机关、党群组织、企事业单位负责人(OR=1.769,95%CI=1.470~2.120)是糖尿病并发症的独立危险因素,而农、林、牧、渔、水利业生产人员(OR=0.647,95%CI=0.520~0.810)和生产、运输设备操作人员(OR=0.902,95%CI=0.833~0.968),以及男性(OR=0.943,95%CI=0.887~1.016)是糖尿病并发症的保护因素。结论疾控中心应重点关注高龄、女性、2型糖尿病患者,以及职业为国家机关、党群组织、企事业单位负责人的人群,为其提供积极干预措施,降低糖尿病并发症的发生风险。 Objective To explore the incidence and influencing factors of diabetes complications in Binhai New Area,and provide evidence for prevention at all levels.Methods A total of 24,697 patients with diabetes mellitus from 7 hospitals in Haibin New District from January 1,2015 to December 30,2017 were enrolled.The patients were divided into complications group(6216 cases)and no complications according to the presence or absence of complications.The group(18481 cases)compared the general data and clinical data of gender,age,ethnicity,occupation,and home address of the two groups,and analyzed the risk factors that may affect the complications of diabetes.Results①The differences in gender,age,occupation,and diabetes diagnosis between the two groups were statistically significant(P<0.05).②Multivariate logistic regression analysis showed that age(OR=1.011,95%CI=1.000~1.020),type 2 diabetes(OR=2.650,95%CI=1.137~6.160)and occupations were state organs,party organizations,enterprises and institutions.The person in charge of the unit(OR=1.769,95%CI=1.470~2.120)is an independent risk factor for diabetes complications,while the production staff of agriculture,forestry,animal husbandry,fishery and water conservancy(OR=0.647,95%CI=0.520~0.810)and production and transportation equipment operators(OR=0.902,95%CI=0.833~0.968),and men(OR=0.943,95%CI=0.887~1.016)are protective factors for diabetic complications.Conclusion The CDC should focus on the elderly,women,type 2 diabetes patients,and the population of the state organs,party organizations,enterprises and institutions,and provide them with positive intervention measures to reduce the risk of diabetes complications.
