多巴胺联合波生坦治疗肺动脉高压及对血清pro-BNP、SOD的影响 被引量:5

Curative effect of dopamine combined with bosentanin on pulmonary arterial hypertension and its influence on serum pro-B-type natriuretic peptide and superoxide dismutase

摘要 目的探讨多巴胺联合波生坦治疗肺动脉高压及对血清B型脑钠肽前体(proBNP)、超氧化物歧化酶(SOD)的影响。方法选择2016年2月至2018年2月于武汉亚洲心脏病医院心内科收治的肺动脉高压患者90例进行研究,分为观察组(n=48)和对照组(n=42)。对照组给予波生坦治疗,观察组在此基础加用多巴胺。比较两组患者的临床疗效、proBNP、SOD、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、FEV1/FVC、动脉氧分压(PaO2)、二氧化碳分压(PaCO2)水平变化情况及不良反应发生情况。结果治疗后,两组总有效率分别为93.75%、73.81%,P<0.05;治疗后,两组患者血清proBNP、SOD水平均改善,且观察组proBNP低于对照组,SOD高于对照组(P<0.05);两组患者肺功能指标较治疗前均显著升高(P<0.05),观察组患者的FVC、FEV1、FEV1/FVC水平均明显高于对照组(P<0.05);治疗后,两组的PaO2、PaCO2水平较治疗前均显著改善(P<0.05),观察组PaO2水平明显较对照组低,PaCO2明显高于对照组(P<0.05)。结论肺动脉高压患者应用多巴胺联合波生坦治疗,可有效改善患者proBNP、SOD水平,减少不良反应。 Objective To discuss the curative effect of dopamine combined with bosentanin on pulmonary arterial hypertension(PAH)and its influence on levels of serum pro-B-type natriuretic peptide(proBNP)and superoxide dismutase(SOD).Methods PAH patients(n=90)were chosen from Department of Cardiovascular Medicine in Wuhan Asia Heart Disease Hospital from Feb.2016 to Feb.2018,and then divided into observation group(n=48)and control group(n=42).The control group was treated with bosentanin and observation group was treated with dopamine combined with bosentanin.The clinical efficacy,levels of serum pro-B-type natriuretic peptide(proBNP)and superoxide dismutase(SOD),level changes of forced vital capacity(FVC),1-second forced expiratory volume(FEV1),ratio of FEV1 to FVC(FEV1/FVC),arterial partial pressure of oxygen(PaO2)and arterial partial pressure of carbon dioxide(PaCO2),and incidence of adverse reactions were compared between 2 groups.Results The total effective rate was 93.75%in observation group and 73.81%in control group after treatment(P<0.05).After treatment,the levels of serum pro-BNP and SOD were improved,and level of serum pro-BNP was lower and level of serum SOD was higher in observation group than those in control group(P<0.05).After treatment,the indexes of lung function increased significantly in 2 groups(P<0.05),and levels of FVC,FEV1 and FEV1/FVC were significantly higher in observation group than those in control group(P<0.05).After treatment,the levels of PaO 2 and PaCO2 were significantly improved in 2 groups(P<0.05),and PaO2 level was significantly lower and PaCO 2 level was significantly higher in observation group than those in control group(P<0.05).Conclusion Dopamine combined with bosentanin can effectively improve the levels of serum pro-BNP and SOD,and reduce adverse reactions in PAH patients.
