摘要
凶险型前置胎是产科的危重病症,是引起孕产妇死亡的重要因素。患者围术期风险极高,必须重视围术期管理,组建多学科的诊治团队合作来共同管理。术前明确诊断,并根据患者情况及影像学资料综合评估患者的病情及手术风险,制定详细的手术计划、麻醉计划及应急策略,做好充分的术前准备。术中各科室分工协作,尽量避免患者出现严重出血、弥散性血管内凝血等危及生命的情况出现,从而改善患者的预后。术后严密观察产后出血情况,避免产后出血致死情况的发生。为此,结合本院经验详述了凶险性前置胎盘患者的围术期管理策略,为广大临床工作者提供参考。
As a critical disease in obstetrics,the pernicious placenta previa is an important factor leading to maternal death.The perioperative risk of patients is very high,so it is necessary to attach importance to perioperative management and establish a multidisciplinary team of diagnosis and treatment to jointly manage.Preoperative diagnosis was clearly made,and the patient's condition and surgical risk were comprehensively evaluated according to the patient's condition and imaging data,detailed operation plan,anesthesia plan and emergency strategy were formulated,and adequate preoperative preparation was made.During the operation,all departments worked together to avoid serious bleeding,disseminated intravascular coagulation and other life-threatening situations,so as to improve the prognosis of patients.Postoperative observation of postpartum bleeding to avoid postpartum bleeding death.For this reason,the perioperative management strategy of patients with pernicious placenta previa is detailed in combination with the experience of our hospital,which can provide reference for the general clinical workers.
