摘要
目的研究社区医疗团队对心身疾病认知及应对能力现况并评价培训效果。方法以2018年上海市部分社区医疗团队为研究对象,通过调查问卷进行现况调查及培训效果评价。结果375名未培训研究对象得分高低依次为精神卫生医师(79.79±11.04),公共卫生医师(75.23±14.65),西医全科医师(73.92±14.47),中医全科医师(68.94±14.50),护士(67.26±14.55)(P<0.01);问卷各模块得分高低依次为基本概念(76.07±15.64),临床应对(70.21±16.05),临床认知(69.67±17.93)和培训内容(66.61±18.77)(P<0.01);培训后总分(75.52±13.51)高于未培训总分(71.23±15.31)(P=0.03);培训后的临床认知和临床应对模块高于未培训(P=0.03;P=0.04)。结论社区医疗团队对心身疾病的认知应对能力存在不足,全科医生精神心理培训效果体现在临床应对和临床认知模块。
Objective To study the current status of community healthcare teams' cognition and coping a-bility to psychosomatic diseases and evaluate the training effect.Methods Some community healthcare teams of Shanghai in 2018 were analyzed to investigate the current situation and training effect by questionnaires.Results The scores of 375 untrained subjects from high to low were mental health doctors(79.79±11.04),public health doctors(75.23±14.65),general practitioners of western medicine(73.92±14.47),general practitioners of tradi-tional Chinese medicine(68.94±14.50)and nurses(67.26±14.55)(P<0.01).The scores of each module in the questionnaire from high to low were basic concepts(76.07±15.64),clinical response(70.21±16.05),clinical cognition(69.67±17.93)and training content(66.61±18.77)(P<0.01).The total score after training(75.52±13.51)was higher than the total score without training(71.23±15.31)(P=0.03).After training,the scores of clinical cognition and clinical coping were higher than those without training(P=0.03;P=0.04).Conclusion The community healthcare team lacks the cognition and coping ability to psychosomatic diseases.The effect of this psychological training for general practitioners is reflected in clinical response and clinical cognition modules.
