摘要
目的探讨氯氮平联合阿立哌唑治疗难治性精神分裂症的临床效果及对血脂和血糖的影响。方法选取2017年1月—2018年12月收治的80例难治性精神分裂症患者为研究对象,依据治疗方法不同分为观察组和对照组,每组40例。对照组予以氯氮平治疗,观察组予以氯氮平联合阿立哌唑治疗,均治疗6周。比较2组的临床疗效以及治疗前后的阳性与阴性症状量表评分、空腹血糖(FPG)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、胰岛素水平和胰岛素抵抗指数。结果观察组的临床有效率显著高于对照组(P<0.05)。治疗后,2组的阳性量表、阴性量表及一般精神病理量表评分均较治疗前下降,且观察组低于对照组(P<0.05)。治疗后,2组的FPG、TC、TG、LDL、胰岛素水平及胰岛素抵抗指数均较治疗前上升,但观察组低于对照组(P<0.05)。结论氯氮平联合阿立哌唑治疗难治性精神分裂症能够显著提高临床疗效,且降低药物所致的糖脂代谢异常等不良反应,可能与改善胰岛素抵抗有关。
Objective To investigate clinical effectiveness of Clozapine combined with Aripiprazole in treatment of patients with refractory schizophrenia and its effects on blood lipid and blood glucose.Methods A total of 80 patients with refractory schizophrenia admitted during January 2017 and December 2018 were divided into observation group and control group(n=40 in each group)according to different therapeutic methods.Control group was treated with Clozapine,while observation group was treated with Clozapine combined with Aripiprazole,and both two groups were treated for 6 weeks.In two groups,clinical efficacy,positive and negative symptom scale scores,fasting blood glucose(FPG),total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL),insulin level and insulin resistance index before and after treatment were compared.Results The clinical effective rate in observation group was significantly higher than that in control group(P<0.05).After treatment,scores of positive scale,negative scale and general psychopathology scale were significantly lower than those before treatment in two groups,and the scores in observation group were significantly lower than those in control group(P<0.05).After treatment,levels of FPG,TC,TG,LDL,insulin and insulin resistance index were significantly higher than those before treatment in two groups,but the levels in observation group were significantly lower than those in control group(P<0.05).Conclusion Clozapine combined with Aripiprazole in treatment of patients with refractory schizophrenia may significantly improve clinical effectiveness,and reduce drug-induced adverse reactions such as abnormal glycolipids metabolism,which may be related to improving insulin resistance.
