某院2012年-2018年低及极低出生体重新生儿罹患坏死性小肠结肠炎的危险因素分析 被引量:4

摘要 目的探讨福建医科大学附属南平第一医院自2010年成立新生儿重症监护病房(NICU)以来,低出生体重儿(LBWI)、极低出生体重儿(VLBWI)(体重<2500 g)发生坏死性小肠结肠炎(NEC)的危险因素。方法2012年1月—2018年12月笔者所在医院NICU收治LBWI、VLBWI共计2291例患儿,根据NEC诊断标准将这些新生儿分为NEC组和非NEC组,收集两组患儿及其产妇临床资料进行单因素统计分析。之后按照相近胎龄和体重1∶1配对NEC组和非NEC组进行多因素Logistic回归分析。结果在NEC组中低胎龄,低出生体重,RDS,肺炎,PDA,布洛芬治疗PDA比例明显高于非NEC组,差异具有统计学意义(P<0.05);同时NEC组产妇产前使用抗生素治疗明显高于非NEC组,差异具有统计学意义(P<0.05)。多因素Logistic回归分析显示:RDS(OR=4.32,95%CI:2.41~16.24),肺炎(OR=7.54,95%CI:3.74~24.41),PDA(OR=2.94,95%CI:1.87~6.23)是LBWI、VLBWI发生NEC的危险因素(P<0.05)。结论低胎龄,低出生体重,肺炎,RDS,PDA,布洛芬治疗PDA,产妇产前使用抗生素治疗,这些因素对LBWI、VLBWI诱发NEC起到重要的作用,增加了LBWI、VLBWI发生NEC的风险,其中RDS,肺炎,PDA是发生NEC独立危险因素。 Objective To explore the risk factors of necrotizing enterocolitis(NEC)in low birth weight infants(LBWI)and very low birth weight infants(VLBWI)(weight<2500g)since the establishment of Neonatal Intensive Care Unit(NICU)in Nanping First Hospital Affiliated to Fujian Medical University in 2010.Methods A retrospective study was conducted on 2291 cases of LBWI and VLBWI admitted to NICU from January 2011 to December 2018.These neonates were divided into NEC group and non-NEC group according to NEC diagnostic criteria.Clinical data of the two groups were collected and analyzed,then multiple conditional logistic regression analysis was performed in matched NEC and non-NEC groups at 1∶1 according to the similar gestational age and body weight.Result Statistics showed that:in NEC group,the proportion of low gestational age,low birth weight,RDS,pneumonia,PDA,ibuprofen treatment of PDA was significantly higher than that in non-NEC group,with statistical significance(P<0.05);at the same time,prenatal antibiotic treatment in NEC group was significantly higher than that in non-NEC group,with statistical significance(P<0.05).Multiple conditional logistic regression analysis showed that high risk factors for NEC included RDS(OR=4.32,95%CI:2.41-16.24),pneumonia(OR=7.54,95%CI:3.74-24.41),PDA(OR=2.94,95%CI:1.87-6.23).Conclusion The factors suchas low gestational age,low birth weight,pneumonia,RDS,PDA,ibuprofen treatment of PDA,prenatal antibiotic treatment,are the risk factors of NEC in LBWI and VLBWI,which increased the risk of NEC in LBWI and VLBWI.RDS,pneumonia and PDA,thus are independent risk factors for NEC.
